Objective:To compare effects of T1 renal carcinoma between Laparoscopic nephron sparing surgery and Laparoscopic radical nephrectomy.Method:A retrospective analysis of 61 T1 cases in The First People hospital of Jining from 2010 to 2015.The patients were divided into LNSS group and LRN group based on surgical approach.The 2 groups were observed and compared with the duration of operation,intraoperative blood loss,postoperative drainage days,hospital days,changes in preoperative and postoperative serum creatinine level and complication.And also to compared with the tumor recurrent,far transferred and cancer-specific death(CSD)in attending years.Results:No significant difference was found in the duration of operation,intraoperative blood loss,postoperative drainage days,hospital days and preoperative serum creatinine level(P>0.05).There were no complication in the two groups.The two groups were survival without tumor,no recurrent was found,no far transferred and no death of carcinoma in attending years.There were obviously differences for the postoperative serum creatinine level,and the LNSS group was lower than the LRN group(P=0.000).Conclusion:In this series,LNSS was associated with improved renal function outcomes with no additional morbidity compared to LRN.Although no difference was noted between overall survival(OS)and cancer-specific survival(CSS),LNSS technique can have greater implication in the T1 stage patients due to reduced baseline kidney function.These data suggest that appropriately selected patients may benefit from LNSS,and tumor diameter should not be a contraindication to LNSS.
