Objective To explore the improved method of repair for macrostomia in order to achieve minimal surgical scars with natural looking commissure.It is designed to further enhance the effect of the macrostomia reconstruction surgery.Methods4 layered improved surgical method was used in our 33 patients in order to aesthetically and functionally correct macrostomia.The upper lip based vermilion square flap is rotated to lower lip to achieve a new commisure.Functional myoplasty was performed to achieve normal anatomical structure.The aestheic subunits and melolabial fold were carefully considered for skin closure for better aestheic outcome.Traditional trilaminar technique was improved.Stitches were taken out after 7 days.Pictures were collected during preoperative,post operative and during follow up.Results Most of the patients had no local infection,bleeding,stage 1 healing was achieved.Maximum time of follow up was 2 years.There was no lateral migration and symmetric lips was observed.Normal oral anatomy was restored with good oral competence.Natural looking commissure with aesthetically pleasing scar was obtained.Conclusion: The modified 4 layered surgical technique is simple in design,easy to replicate and aesthetically and functionally corrects the macrostomia.
