Background and Objective:Primary liver cancer(PLC)is one of the most prevalent malignancies with high morbidity and mortality in the world.At the present stage,there are many shortcomings such as difficulty in biopsy,certain trauma and complications,low sensitivity,radiation,and high price for clinical diagnosis of liver cancer.In this study,the SE-iFISH technique was used to perform preliminary detection of various subtypes of circulating tumors(CTCs)in PLC patients,in order to analysis the relationship between CTCs with other indexes of PLC.The clinical value of CTCs detection in early detection of PLC and prognosis of clinical prognosis was preliminarily discussed.Methods:We Collected 20 patients with primary liver cancer who were admitted to the hospital for surgery at the Department of Hepatobiliary Surgery,Beijing Tsinghua Changgung Hospital from November 2015 to June 2016.Before and a week after surgery,the patients were collected 6mL of peripheral blood.The SE-iFISH technique was used to separate and identify circulati tumor cells.At the same time,the pathological diagnosis results and clinical data of each patient were collected and recorded to predict the correlation between the clinical pathological features of CTCs and the prognosis of liver cancer.Results:①The median number of CTCs before surgery was4 per 6 mL of peripheral blood,with an average of 4.77±3.07 and on the 7th day after surgery was 10 per 6 mL of peripheral blood,with an average of 10.27±6.29.② The number of diploids in chromosome 8 aneuploidy was 75(31.25%),the number of tetraploids was 55(22.91%),and the number of ploidyloids was 128(53.33%).③The number of CTCs in peripheral blood was significantly higher than that before surgery(p<0.05).The number of CTCs in peripheral blood before surgery was significantly related to the concentration of AFP and the presence of vasculature(p<0.05).The number of CTCs in peripheral blood was significantly associated with hepatitis virus infection,vascular cancer embolism,and TNM staging after one week of surgery(p<0.05).Conclusions:①The number of CTCs in peripheral blood of patients was significantly higher than that before surgery in one week after surgery,indicating that surgery can cause distant metastasis of cancer cells.② The specific subtype of small CTCs with chromosome 8>5-ploid is another key player in the metastasis and recurrence of cancer in PLC patients.Its postoperative number has a very significant correlation with poor prognosis of PLC patients.③ After resection of recurrent PLC patients after surgery,the specific subtypes of ≥5-ploidy CTCs were increased,which has a very significant correlation with the poor prognosis of PLC patients.
