Objective Bladder transitional cell carcinoma is the most common malignant tumor of the genitourinary system, the high recurrence rate is its most prominent features. It is the key of improve treatment effect to early diagnosis, early detection of recurrence, prompt treatment. Frequently, the existence of chromosomal aberrations in bladder transitional cell carcinoma, Multicolor Fluorescence in Situ Hybridization (M-FISH) is a relatively new technology to test chromosome aberration. Our study, To evaluate the possibility and validity of detecting numerical aberration of chromosomes 3, 7, 9 and 17 by multicolor fluorescence in situ hybridization (M-FISH) in order to diagnose bladder transition cell carcinoma (BTCC), and tried to prove that the centromere of chromosome 3,7,17 and 9P21 zone probe and their combination has important significance in clinical diagnosis of the Chinese bladder cancer patient. To establish the chromosome group to suited to the Chinese people and able to use FISH technology to assist diagnosis of bladder cancer in the clinical. At the same time, for molecular cytogenetic study technology (M-FISH) laying the groundwork in the clinical auxiliary diagnosis of bladder cancer and better serving for a large number of patients in the future.Methods A total of 66 volunteers were recruited as subjects, including 46 patients suffering from BTCC, 15 non-tumor patients and 5 healthy individuals. Exfoliated cells from fresh urinary specimen was collected and examined by M-FISH using centromeric probes of chromosome 3,7,17 and region probe of 9p21. At the same time, Cytology was applied and all of the patients were definitely diagnosed by Pathology. To compare their sensitivity and specificity,to analysis the M-FISH detection'positive rate of chromosome aberrations at exfoliated urothelial cells of different stage and grade of bladder cancer and to compare the correlation of various chromosomal aberrations and malignant degree by bilateral x2 exact test methods of the SPSS 11.5 statistics software.Resultsâ‘ The sensitivity and specificity of M-FISH and cytology on detecting bladder cancer were 73.9%,47.83% and 95%,100%, respectively.â‘¡That sensitivity of M-FISH in Ta-T1 and T2-T4 patients was 50%,84.38%, respectively. That sensitivity of M-FISH in G1,G2,G3 patients was 42.86%,80%,100%, respectively. The sensitivity was associated with the clinical stage and pathological grade.â‘¢The frequency of numerical aberration of chromosomes 3,7,17 and 9 was 71.3%,73.9%,73.9% and 56.52% in urinary exfoliated cells, respectively. Abnormality of chromosomes 3, 7and 17 was associated with the clinical stage and pathological grade. But aberration of chromosomes 9 was not correlated with pathological grade of cancer and stage of the disease. Conclusion The sensitivity and specificity of M-FISH detecting 3, 7, 9, 17 chromosome aberrationns are highly in the diagnosis of bladder cancer. It is a rapid, noninvasive, accurate and very sensitive method. The technology has important clinical value to diagnosis, early diagnosis, degree of malignancy, development trend and monitoring of prognosis of bladder transitional cell carcinoma.
