Assistant Microwave Ablation Improve The Prognosis Of Hepatocellular Carcinoma Patients Undergoing Liver Resection

Objective.We evaluated microwave(MW)-assisted liver resection for hepatocellular carcinoma(HCC).Methods: the clinical data of 79 patients with HCC were collected from January 2015 to June 2015.Patients were randomly divided into group A(pure surgery treatment group 40 cases,50.6%),not using microwave ablation in the liver resection,group B(combined treatment group 39 cases,49.4%),assisted with microwave ablation in the liver resection.Two groups of patients were collected,including preoperative,intraoperative and postoperative conditions.Mainly include the patient's basic information(sex,age,etc.),etiologic factors of viral hepatitis,liver function index(TBIL,ALT,propagated,GGT,Albumin),AFP,prothrombin time(PT),imaging(CT or MRI),postoperative complications,the length of time and follow-up data,etc.In addition,the patient's surgical data(tumor number,location,size,operation time,intraoperative bleeding and blood transfusion volume,etc.)were collected,and the data of the two groups were statistically analyzed.P < 0.05 was considered statistically significant.Results: in all the research object,we observed that in the two groups of patients with gender,age(average age was 59.32 ± 10.34)and tumor average diameter(4.39 ±2.00 cm)these data have no obvious differences between the two groups.In the combined treatment group,there were 10 cases with more than 200 ml of blood loss,which was less than 28 cases in the surgical treatment group,and the difference was statistically significant(p<0.001).Postoperative day 1(P=0.005)and day 3(P=0.019)of the combined treatment group was significantly less than that of the operation group.In addition,there were 13 patients(33.3%)who were hospitalized for more than 10 days after the operation of the combined treatment group,which was also significantly lower than the 33 cases(82.5%)in the surgical group(82.5%),(p<0.001).We found in the late follow-up in the combination group in 4 patients(10.2%)of tumor recurrence,less than the 11 cases of group A(p = 0.047),and the joint treatment group happen distant metastasis of the tumor is less,but no statistical significance(p = 0.061).In terms of operation time,47.5% of patients with group A were over 3 hours,and 48.7% in group B,with no statistically significant difference.The cumulative survival rate of the patients in the two groups was compared with that in the three years: the 3-year survival rate was 47.5%,which was less than 66.7% of the combined treatment group,and the difference was statistically significant(P=0.044).Conclusion: the therapeutic effect of combined microwave ablation is better than that of simple surgical excision.
