Comparison Of Gastric Emptying And Comfort After Intake Of Different Carbohydrate Drinks In Healthy Volunteers

Objective:1.Use ultrasonography to observe the evacuation of stomach on healthy volunteers after they have ingested fresh water and 2 types of carbohydrates.2.Compare the comfort levels after healthy volunteers have ingested the above liquids.Methods:A total of 18 healthy volunteers aged from 18 to 45 years were selected,and the male to female ratio was 1:1,ASA classification I-II,body mass index ranged from19 to 25kg/m~2,excluding patients with pregnancy,diabetes,metabolic diseases,kidney disease and organic digestive diseases.Each volunteer was to drink 300 ml three different liquids randomly in three weeks:clear water(energy:0 kcal,PH=7.0,viscosity:0.69mPa·s),carbohydrate drinks NO.1(osmotic pressure:280mOsm/L,energy:723kJ,sugar:41.4g,amino acid:150mg,PH=4.9,viscosity:1.95mPa·s,trade name:Shuneng),carbohydrate drinks NO.2(240mOsm/L,energy:648kJ,sugar:36.3g,PH=4.2,viscosity:1.63mPa·s,trade name:Suqian).Then ultrasound images of gastric antrum were obtained from patients taking horizontal position and right lateral position respectively.Anterior-posterior diameter and head-tail diameter of gastric antrum were recorded before and immediate(3min),15 min and 30 min and 60 min,90min and 120 min after drinking the three different fluids,The longest diameter of gastric antrum image from AP,CC were taken on the base of AP and maximum value was obtained by using AP.The meridian points from one side serosa to contralateral serosa layer,while measuringand cross-sectional area(CSA)and gastric volume(GV)were calculated by the formula.Visual analogue scale(VAS)was used to record degrees of hunger,thirst,anxiety and dry mouth in fasting and immediately(3min)after drinking,after 30 min,60min and 90 min respectively.Results:(1)After intake of different CHO or clear water,CSA of stomach increased instantly and declined gradually after 15 min.But the CSA and GV of stomach filled with clear water returned back to baseline value in fasting state within 60 min,but the CSA and GV of stomach intake CHO drinks NO.1 returned to baseline after 90 min,and he CSA and GV of stomach intake CHO drink NO.2 returned to baseline after 120 min.(2).After three different liquids were ingested,the ratio of gastric juice/body weight was reduced to<1.5ml/kg in the clear water group within 60 minutes,and the ratio of gastric juice/body weight was reduced to<1.5 ml/kg in the CHO NO.2 beverage group within 90 minutes,while in the CHO NO.1 beverage group was reduced to<1.5 ml/kg within 120 minutes.(3)Thirsty,anxiety,hunger and dry mouth have been relieved in varying degrees after intake of three different liquids at each time point,but compared with clear water,discomfort symptoms such as thirst,anxiety and hunger and dry mouth of volunteers have been relieved better after intake of two kinds of CHO drinks,and the differences of comparing VAS scores was statistically significant.Conclusions:This study found that all healthy volunteers,gastric emptying happened in 2 hours after drinking clear water or other two drinks rich in carbohydrates,speed of gastric emptying was as follow:clear water,CHO NO.1and CHO NO.2,The three groups of liquids intake can be reduced to the clinically assessed risk threshold of regurgitation and aspiration within 2 hours(gastric volume to body weight ratio?1.5ml/kg),the order are:clear water.CHO NO.2.CHO NO.1.which hinted that hit was safe for healthy volunteers to drink CHO drinks 2 hours before anesthesia induction.Meanwhile CHO beverage can greatly reduce subjective feelings such as degree of thirst,hunger,anxiety and dry mouth caused by fasting.
