Objective:To compare and analyze the clinical application and effect of laparoscopic surgery and laparotomy in the treatment of hepatic hemangioma.Methods:Through the inclusion criteria and exclusion criteria,in January 2014-December 2015,line on hunan province hospital liver surgery clinical medical records of 166 patients with hepatic hemangioma were retrospectively analyzed.According to the different methods of operation are divided into laparoscopy group(group A)and laparotomy group(group B),in which A group is divided into A1(strip)hepatic hemangioma(liver resection)and A2 group,group B is divided into B1(strip)hepatic hemangioma and group B2)(liver resection.According to the operation time two groups of cases,liver cutting time,intraoperative blood loss,postoperative complications,postoperative hospital stay and postoperative recurrence 2 years follow-up were analyzed to determine the surgical effect.Results:Among the 166 patients in the whole group,92 cases of laparoscopic surgery group(group A)were performed,among which 75 patients underwent laparoscopic hemangioma removal(A1 group),and 17 cases were performed under laparoscopic hepatectomy(A2).Among the 74 patients in the open surgery group(group B),there were 60 cases of laparotomy(group B1)and 14 cases of laparotomy(B2).The operation time of group Al was 60?260 minutes,with an average(110.0 + 41.3)minutes.The time of cutting liver was 45?240 minutes,averaging(95.0 ++ 42.2)minutes;Intraoperative blood loss of 50?800 ml,average(290 +120)ml;Postoperative hospitalization time was 4?9 days,with an average(6.3 + + 2.4)days.The operation time of group A2 was 66?300 minutes,with average(125.0 + + 40.1)minutes.The time of cutting liver was 50?280 minutes,with an average(110.0 + 39.7)minutes,intraoperative blood loss of 100?900 ml,average(300 + 130)ml;Postoperative hospitalization time was 5?18 days,with an average(7.2 + + 2.9)days.Among them,the hepatotomy time of A1 group was significantly longer than that of group B1,and the liver time of group A2 was significantly longer than that of B2 group.The postoperative hospitalization time of A1 group was significantly lower than that in group B1,and the postoperative hospitalization time of group A2 was significantly lower than that in group B2,and there was no significant difference in the duration of surgery,intraoperative blood loss,postoperative complications and recurrence rate.Full set of operation are successful,no perioperative death cases,there was no serious postoperative complications,the hospital successfully,symptomatic patients with symptoms disappeared basically,postoperative liver hemangioma recurrence 2 years follow-up.Conclusions:1.Laparoscopic surgery and laparotomy is a reliable method of treatment of liver hemangioma,complete resection can achieve effect a radical cure,curative effect,can be applied to different parts and different size to the treatment of liver hemangioma,and lower complications related to the operation chance and have high security.2.Compared.with open surgery,laparoscopic surgery has little trauma and postoperative recovery in the treatment of hepatic hemangioma,and there is no significant difference in safety and reliability,and it is worthy of clinical promotion.3.In the clinical application,it is also necessary to strictly control the operation pointer to avoid excessive medical treatment,and the correct choice of surgical procedures can reduce the risks associated with the operation.
