Shandong Provincial Government Health Expenditure Efficiency And Influencing Factors

As a public enterprise,medical and health service matters vital to national well-being and the people’s livelihood.In recent years,China has made great progress in medical and health care,but there are still many problems.The effectiveness of government health expenditure directly affect the public health service level.The scale of government health expenditure in shandong province is increasing year by year,and its growth rate is greater than the growth rate of public budget expenditure.To study the efficiency of government health expenditure and its influencing factors will make an important contribution to improve health service level and people’s living standard.Firstly,this paper makes a theoretical analysis of the efficiency of government health expenditure,and defines the related concepts.Government health expenditure is the funds used by various levels of government for various health services;The core of government health expenditure efficiency is the effectiveness of government health expenditure.Then the article analyzes the current situation of Shandong provincial government health expenditure.Firstly,the scale of expenditure was analyzed from the absolute scale and relative scale of the health expenditure of the Shandong provincial government.The situation of health resources and health services were analyzed.It was found that the scale of health expenditure of the Shandong provincial government has increased year by year.The city’s health care expenditures and health resources and medical and health services are quite different.After that,the paper focuses on the empirical analysis of the efficiency and influencing factors of the government health expenditure in shandong province.The government medical expenditures,number of beds,and number of health institutions in 17 cities were selected as input indicators.The number of diagnosed and treated patients,the emergency mortality rate,the ratio of the number of discharged patients to the number of hospital admissions,and the utilization rate of beds were used as output indicators.The DEA model was used for medical treatment.The efficiency of health expenditure has been measured and analyzed.The results show that,from a time perspective,the efficiency of health expenditure of the Shandong provincial government has generally been on an upward trend,and has continued to rise after a slight decline in 2012;from the perspective of regional differences,the efficiency gap of government health expenditure in various cities in Shandong Province Larger and still more government spending on health care is less efficient.Specifically,each city has high pure technical efficiency,and the trend of the average of integrated technical efficiency and scale efficiency is roughly the same.This shows that the trend of rising and fluctuations in integrated technical efficiency is affected by the scale efficiency,and all cities’ The government’s health care expenditures are all in the stage of diminishing returns to scale.For these cities,the scale of expenditure can be adjusted on the basis of raising the level of capital management,and the government’s investment in health care can be appropriately reduced to improve efficiency.In the analysis of the influencing factors,the article constructed the Tobit model,and selected seven influencing factor indicators for empirical analysis.The results show that fiscal decentralization is negatively related to the efficiency of government expenditure on health care,and the proportion of government expenditure on health care as a percentage of GDP.There is a positive correlation between the Engel coefficient of urban residents and the level of urbanization and the efficiency of government expenditure on health care.There is no correlation between the per capita GDP and rural residents Engel coefficient,population density and the efficiency of government health care expenditure.One of the factors that has the greatest impact on the efficiency of government health care expenditure is the proportion of government expenditure on health care as a percentage of GDP.According to the above article on the efficiency of government health expenditure in shandong province and its influencing factors of the empirical analysis,Finally,the paper respectively from improve the fiscal decentralization system,optimize the structure of government health expenditure,accelerating urbanization and narrowing the Urban-Rural income gap,and other suggestions including accelerate the reform of medical and healthsystem,strengthen the supervision of government health expenditure budget,to set up the evaluation system for government health care spending four aspects put forward the policy suggestions of improving efficiency.
