Study On The Rule Of Thyroid Nodule Syndrome And The Outcome Report Scale Of ChineseMedicine Patients

Part 1:The rule of thyroid nodule syndromeObjective:To explore the syndrome differentiation,treatment and common drugs of thyroid nodule by sorting out and excavating clinical cases.Methods:All the patients included in this study were thyroid nodules in the endocrine ward and outpatient clinic of Guang AnMen hospital,Chinese academy of traditional Clhinese medicine.from January 2016 to December 2017,totaling 180 cases.Text information extraction was carried out for all the included cases,including the results of syndrome differentiation,symptoms,medication,etc.,and the establishment of original information database for summarization and statistics respectively.Text information using SAS programming import database,refer to the internal medicine of traditional Chinese medicine,diagnostics of traditional Chinese medicine,Chinese traditional surgery,journal of Chinese materia medica normalized description of materials,specifications in the cases of extracting information of disease,symptoms,drug name,the text after the specification and counting information input Microsoft office excel,establishing composition table,symptoms in patients with type TAB,drug summary table,the TAB.Using Spss 24.0 to extract the count of data frequency,frequency statistics,symptoms count material clustering analysis and the main factors of extraction,and combining with Spss modeler 18.0 the drug count data of frequency statistics after correlation analysis.Analyze the statistical results.Results:(1)From the analysis of the frequency of symptoms,the result of system cluster analysis and principal factors,the type of thyroid nodules often witness is divided into five categories including:liver depression TanNing,primary symptom is:the neck disease bilges frowsty,along with the mood swings,bosom frowsty,poor mood,good heave a deep sigh,pharyngeal foreign body sensation,pulse string with whitish and greasy coating;Two deficiency of qi Yin,the main symptoms:palpitation of the heart,weak waist and knees,night sweats.five heat upset,hot and sweaty,insomnia,more dreams,dry mouth,dry throat,thin pulse;The main symptoms are:fear of heat and sweat.irritability,irritability,irritability,red tongue,bitter mouth,liver-yellow,pulse number.Phlegm and blood stasis are mutually knotted,and the main symptoms are:neck suffocation and tingling pain,flank and rib tingling pain,dark red tongue,bruising and bruising tongue.Spleen and kidney Yang deficiency,the main symptoms are:pale face,warm limbs,loose stools,tolerance,floating limb swelling,god tired and idle,less gas lazy speech,late pulse.(2)Cases of this study of the communist party of China 16 class of drugs,including a higher frequency of the top 10 medicine is:phlegm,throat medicine,qi,promoting blood circulation to remove blood stasis drugs,functions of medicine,clear heat,qushi medicine,Yin,Yang,tonifying qi tonic medicine,sedative drugs.The common methods of treatment include removing phlegm by qi,removing phlegm,removing phlegm and removing stasis,removing phlegm and removing wind,removing heat and removing wind,removing heat and removing wind,nourishing both qi and Yin,reducing phlegm by clearing damp,warming phlegm and removing stasis,and carrying out air temperature and Yang.(3)Through the use of high frequency of the drug on the jin,thunberg shell and frankincense,thunberg shell and oyster,thunberg shell and curcuma.Conclusion:Thyroid nodules can be divided into liver depression phlegm,sputum stasis,qi Yin deficiency,liver fire,spleen and kidney Yang deficiency by information mining of clinical cases.The commonly used drugs for removing phlegm,regulating qi and dispersing qi,activating blood and removing bloocd stasis are:thunberg shellfish,dried grass,bupleurum chinensis,yu jin,curcuma curcuma,three edges,half summer,oysters,fructus trichosanthes,and shanci mushroom.Commonly used pairs of medicines are:thunberi and salicus edodes,thunberi and pinellia sinensis,thunberi and cat-claw,thunberi and white mustard seed,yu jin and bupleurum chinensis.three edges and curcuma.Zhe bei and bupleurum,zhe bei and yu jin,zhe bei and frankincense,zhe bei and oyster,zhe bei and zedoary.Part 2:The study on the PRO scale of thyroid nodule medicineObjective:Under the guidance of the theory of traditional Chinese medicine,the outcome report scale of thyroid nodule patients in traditional Chinese medicine was established to provide references for the clinical curative effect of thyroid nodule.Methods:A scale study group was established.Determine the characteristics of the target scale.Based on TCM etiology and pathogenesis,clinical manifestations of thyroid nodule using viscera qi and blood,eight principles and syndrome differentiation of Yin and Yang,combining the theory of scale development of a theory framework of patients with thyroid nodule result report scale.Under the guidance of the theoretical framework,a questionnaire entry pool was established by means of literature review,case analysis,patient prograrm and expert consultation.By the team members to preliminary screening of the item pool,initially formed the item pool based on the index entry in the item pool,developed symptoms of corresponding entries,in the reaction after scale and scale basic format,for patients with cognitive tests,according to the result of test of the length of the scale,such as language accordingly adjusted,form the primary scale.According to the stratified sampling method to extract the inventory report theoretical framework.Formation of the primary scale has 35 items,covering four areas,nine aspects,including physical field item 21,psychological field entry eight,independence field item 3 and social field item 3.In a small range of clinical trials,a total of 200 scales were distributed according to the principle of stratified sampling,200 of which were recovered,and the recovery rate was 100%.The final selection scale formed has 30 items,including 4 fields and 9 aspects.The Cronbach's pruning coefficient of the general table is 0.892.The Cronbach's q factor in physiological field was 0.816.The Cronbach's q factor in the psychological field is 0.701.In the independent field,Cronbach 's processors coefficient is 0.775.The Cronbach's q factor in the social field is 0.756.Half-fold reliability:spearman-brown coefficient,overall 0.8 88;The physiological field was 0.834.The psychological field was 0.729.Independent field is 0.931;The social domain was 0.846.Content validity in the field of entry and the scores,and other various fields score after correlation analysis,the results showed that scale each entry field with its correlation coefficients were greater than 0.4,the prompt scale has good content validity;After factor analysis,5 main components were extracted,and more than 91%of the items could be interpreted by the extracted common factors,indicating that the scale had good structural validity.Conclusion:The PRO scale developed in this study has good reliability and validity and can be used to evaluate the clinical efficacy of thyroid nodule.
