Objective:To evaluate the clinical efficacy of traditional Chinese medicine wax therapy in the treatment of rheumatoid arthritis with cold dampness and stasis syndrome based on the patient's reported outcome. Methods:A total of 72 patients who met the diagnostic criteria of Western medicine RA and DAS28 scores>2.6 and TCM cold and dampness syndrome were enrolled. The patients were divided into two groups by random number table method,36 patients in the experimental group and 36 patients in the control group. The treatment group was the basic treatment plus Chinese medicine wax treatment(once a day),and the control group was the basic treatment plus loxoprofen sodium(one tablet per time, three times a day)for a total of two weeks.Safety indicators(heart rate, respiratory rate),erythrocyte sedimentation rate,TCM symptom score, visual analog pain score(VAS),HAQ scale, and patient report outcome scale were used to evaluate the improvement of clinical symptoms and physiological functions of each group. The evaluation of pain efficacy, the evaluation of DAS28 efficacy, and the evaluation of the main symptoms of TCM were used to evaluate the therapeutic effect.Observe and record adverse reactions during treatment.Result:Safety indicators:There was no statistically significant difference in respiratory rate and heart rate between the two groups before treatment. After treatment, both groups had no effect on respiratory rate and had an effect on heart rate. The heart rate between the two groups was statistically significant after treatment(P<0.05),but the heart rate was in the normal range,so the two groups had no effect on the heart rate.Laboratory indicators:erythrocyte sedimentation rate,C,reactive proteinThere was no significant difference in erythrocyte sedimentation rate and CRP between the two groups before treatment. Both groups had the effects of lowering ESR and CRP after treatment. There was no significant difference between the two groups after treatment(P>0.05),indicating that the two groups had the same effect in reducing erythrocyte sedimentation rate and CRP.Observation indicators:(1)Patient report outcome scaleThere was no statistical difference in the outcome scales between the two groups before treatment. After treatment, both groups of treatments were able to reduce the patient's reported outcome scale. There was a statistically significant difference between the groups after treatment(P<0.05),indicating that the Chinese medicine wax group was superior to the loxoprofen sodium group in reducing the patient's reported outcome scale.(2)DAS28score,VAS score,HAQ scale scoreThere was no statistically significant difference between the two groups in the DAS28 score,VAS score and HAQ scale before treatment. After treatment, both groups had significant effects in reducing DAS28 score, improving pain and improving quality of life. There was no significant difference between the two groups after treatment(P>0.05),indicating that the two groups had the same effect.(3)TCM symptom scoresThere was no statistically significant difference in TCM syndrome scores between the two groups before treatment. Both treatments after treatment were effective in improving TCM symptoms. The difference between the two groups after treatment was statistically significant(P<0.05),indicating that the traditional Chinese medicine wax treatment group was superior to the loxoprofen sodium group in improving TCM symptoms.Efficacy evaluation indicators:(1)evaluation of pain efficacyThe evaluation of pain in the two groups was compared.Among them,36 patients in the traditional Chinese medicine wax treatment group were effective after treatment,6cases were effective,30 cases were effective, and the total effective rate was 100%;36 cases of loxoprofen sodium tablets group were markedly effective,2cases were effective.Effective in 34cases,the total effective rate is 100%.The pain evaluation of the two groups was obtained by chi-square test and P=0.134>0.05.There was no statistical difference, indicating that there was no significant difference between the two groups in improving the efficacy of pain.(2)Evaluation of the efficacy of TCM symptomsAfter a course of treatment(two weeks),the Chinese medicine wax group and the loxoprofen sodium group were equally effective, but the Chinese medicine wax group was superior to loxoprofen in improving the symptoms of Chinese medicine and joint morning stiffness.Sodium group(P<0.05).After 2 weeks of treatment, the effective rate of the Chinese medicine wax treatment group reached 100%,while the significant efficiency of the loxoprofen sodium group was 97.2%.Conclusion:The traditional Chinese medicine wax treatment group and the loxoprofen sodium group have similar clinical effects in the treatment of rheumatoid arthritis patients with rheumatoid arthritis. In the Chinese medicine symptom, the Chinese medicine wax treatment group is better than the loxoprofen sodium group.
