Objective: A retrospective study was conducted to compare the survival differences between radiotherapy combined with chemotherapy and chemotherapy alone in patients with stage ?c nasopharyngeal carcinoma,and to explore whether radiotherapy could delay the progression of patients with stage IVc nasopharyngeal carcinoma and improve the overall survival time.Methods:Between January 2011 and December 2012,the recor ds of 60 patients diagnosed with metastatic nasopharyngeal carcino ma treated in the Hunan Cancer Hospital were included in this stud y.According to the different treatment modalities,the patients were divided into radiotherapy combined chemotherapy group and chemot herapy alone group,with 30 patients in each group.Two groups of patients were treated by different treatment methods,followed by a return visit and data collection.Short term effect was evaluated with RECIST in each group.We applied SPSS18.0 statistical software to evaluated efficacy of each group.Kaplan-Meier and COX regressionanalysis were used to compare OS and PFS,We set statistical signi ficance with P<0.05.Results:The five year progression free survival rate of the w hole group stage ?c nasopharyngeal carcinoma was 16.7%,and the median progression free survival time was 29 months.While the fi ve year overall survival rate was 26.7%.The median overall surviv al time was 35 months.The short-term efficacy showed the respons e rate of the radiotherapy combined chemotherapy group was 63.3%,while the chemotherapy alone group was 56.6%.The difference bet ween the two groups was not statistically significant.The long-term efficacy showed the five year progression free survival rate of the r adiotherapy combined chemotherapy group was 20%,while the chem otherapy alone group was 16.7%.The five year overall survival rat e in the radiotherapy combined chemotherapy group was 30%,while the chemotherapy alone group was 26.7%.There was no statistical difference between the groups.The results of prognostic factors show ed that sex,age,T staging and N staging did not affect the risk fa ctors for survival,and liver metastasis was an independent risk fact or.Conclusion:1.On the basis of chemotherapy,radiotherapy can not improve progression free survival and overall survival time in patients withstage IVc nasopharyngeal carcinoma.2.Gender,age,T staging and N staging were not the risk factors for survival in stage IV C nasopharyngeal carcinoma,while liver metastases were independent risk factors.
