Objective:To investigate the safety of sentinel lymph node biopsy in breast cancer after neoadjuvant chemotherapy,and its relationship between sentinel lymph node metastasis and non-sentinel lymph node metastasis in breast cancer patients after neoadjuvant chemotherapy and relevant clinicopathological features.As to provides a theoretical basis for the reasonable treatment of armpit.Methods:This study is a retrospective review of 82 women with breast cancer after neoadjuvant chemotherapy and underwent sentinel lymph node biopsy from November 2009 to December 2017 in the First Hospital of Jilin University.In the 82 patients,if the sentinel lymph node is negative,the axillary lymph node dissection will not continue,and if the sentinel lymph node is positive,the patients would receiv axillary lymph node dissection.The patients are divided into sentinel lymph node positive group and sentinel lymph node negative group based on the postoperative pathological results of axillary lymphatic state,and divided into non-sentinel lymph node positive group and non-sentinel lymph node negative group based on the postoperative pathological results of axillary lymphatic state.SPSS software were employed to analyze the clinicopathological information.Chi-square test(Fisher test was used if necessary)and Logistic regression analysis has been used to analyze the inter group difference and the risk factors of sentinel lymphatic metastasis and non-sentinel lymphatic metastasis..The statistical results with P<0.05 had statistical significance.Results:There are 18 cases(22.0%)who's sentinel lymphatic state is positive in the 82 cases.6 cases(33.3%)who's non-sentinel lymphatic state is positive in the 18 cases.In univariate analysis,PR before neoadjuvant chemotherapy and PR after neoadjuvant chemotherapy and tumor size after neoadjuvant chemotherapy and pathological type were significant in sentinel lymphatic metastasis.In the multivariate analysis,PR before neoadjuvant chemotherapy and PR after neoadjuvant chemotherapy and tumor size after neoadjuvant chemotherapy and pathological type remained firmly associated with sentinel lymphatic metastasis(P=0.028,OR=2.226(95% IC=1.520-4.007);P=0.024,OR=1.251(95% IC=1.056-2.385);P=0.008,OR=4.685(95% IC=2.546-8.743);P=0.032,OR=0.518(95% IC=0.321-0.939)).Tumor size after neoadjuvant chemotherapy was significant in non-sentinel lymphatic metastasis.The recurrence rate of this study was 2.56%.Conclusion:PR before neoadjuvant chemotherapy and PR after neoadjuvant chemotherapy and tumor size after neoadjuvant chemotherapy and pathological type are the independent risk of sentinel lymphatic metastasis in breast cancer after neoadjuvant chemotherapy.Tumor size after neoadjuvant chemotherapy was significant in non-sentinel lymphatic metastasis.The recurrence rate of sentinel lymph node biopsy after neoadjuvant chemotherapy in this study was lower than reported,It is consistent with the view that sentinel lymph node biopsy is safe and feasible after neoadjuvant chemotherapy.
