Objective: Through the detection of serum C1q/tumor necrosis factor related protein(CTRP9)and IL – 6 in type 2 diabetes mellitus,T2DM)patients,First,to investigate the correlation between serum CTRP9 and obesity,chronic inflammation in T2 DM patients and insulin resistance.Second,to explore the correlation between serum CTRP9 and serum lipid metabolism,the correlation of uric acid metabolism.This provided a theoretical basis for the prevention and treatment of metabolic diseases such as obesity and T2 DM.Methods: The fasting serum CTRP9 levels were measured in 135 T2 DM patients(1999 WHO diagnostic standards)and 45 healthy individuals in our hospital endrocrine department,including 45 cases of T2 DM with normal body weight(group1,20 males and 25 females),45 cases of T2 DM complicating overweight(group2,24 males and 21 females),45 cases of T2 DM complicating obesity(group3,23 males and 22 females)and 45 individuals undergoing the healthy physical examination(group4,25 males and 20 females).Laboratory tests included glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA IR)was calculated,Blood lipid indexes such as triglycerides(TG),total cholesterol(TC),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),Serum uric acid(SUA)were also measured and the determination of serum CTRP9,serum interleukin – 6(IL-6).SPSS19.0 statistical software was used for data processing,analysis of variance(ANOVA)was used to show the difference between the four groups.The relationship between CTRP9 level,IL-6level and other clinical indexs were used by Pearson correlation analysis and multivariate linear stepwise regression analysis.the difference of statistical significance if P<0.05.Result: 1.Comparaed with patients in T2 DM with normal body weight,T2 DM with overweight and control group,the mean values level of BMI,FBG,HOMA-IR were significantly higher in T2 DM complicated with obesity group(P<0.05).Compared with the control group,the FBG,FINS,HOMA IR,HbA1c?TG,TC,LDL-C were higher in other three groups(P<0.05),and HDL – C was lower(P<0.05).FINS,HOMA-IR,TG,LDL-C,and SUA levels in the four groups were successively increased according to healthy control group,T2 DM with normal body weight group,T2 DM complicating overweight group,and T2 DM complicating obesity group(P < 0.05).2.According to the trend test,the level of CTRP9 in the T2 DM complicating overweight group and T2 DM obesity group was significantly lower than that of T2 DM group with normal body weight and healthy control group(P < 0.05).3.Trend test shows that IL-6 levels increased gradually in the control group,T2 DM group with normal body weight,T2 DM complicating with overweight group and T2 DM complicating obesity group,and the difference between every groups had statistically significance(P < 0.05).4.Linear correlation analysis showed that serum CTRP9 level was negatively correlated with BMI?FINS?HOMA-IR?TG?SUA?IL-6,and positively correlated with HbA1 c,HDL-C(P<0.05)and serum IL-6 level was positively correlated with BMI?TG and SUA.5.BMI and TG are independent risk factors of CTRP9;BMI is independent risk factor of IL-6.Conclusion: 1.Compared with patients with T2 DM in normal body weight,insulin resistance and glucose and lipid metabolism disorders were more serious in T2 DM with overweight and obesity.2.The level of serum CTRP9 in patients of T2 DM combined with obesity group was significantly lower than that of healthy control group and T2 DM group with normal body weight,suggesting that CTRP9 may play a role in the occurrence and development of diabetes and obesity.3.Serum CTRP9 level is closely related to obesity,T2 DM insulin resistance,T2 DM chronic inflammation,lipid disorder,Serum uric acid elevation.BMI and TG were independent risk factors for CTRP9 in patients of T2 DM.
