Clinical Observation On Treatment Of Stage? And ? Idiopathic Membranous Nephropathy(Spleen Kidney Yang Deficiency Type) With Jianpi Bushen Prescription

OBJECTIVE: By observing the clinical efficacy of Jianpi Bushen recipe in treatment of stage I and II idiopathic membranous nephropathy(spleen-kidney yang deficiency type),compare the TCM syndrome scores before and after treatment,24-hour urinary total protein(24h-UTP),albumin(ALB),total cholesterol(CHOL),triglyceride(TG),blood urea nitrogen(BUN),creatinine(CREA),alanine aminotransferase(Alanine)Aminotransferase(ALT),to explore its efficacy and safety,to provide some ideas and methods for the study of the disease in the future.METHODS: The study selected 60 inpatients from Henan Provincial Hospital of Traditional Chinese Medicine from April 2017 to December 2017.The 60 patients enrolled were randomly divided into two groups: 30 in the experimental group and 30 in the control group.The control group received basic treatment and hormones.In combination with cyclophosphamide treatment,the experimental group was treated with Jianpi Bushenfang on the basis of the control group.The treatment period was 3months.TCM syndrome scores and laboratory indicators of patients before and after treatment were recorded and monitored(24h-UTP,ALB,CHOL,TG)and safety indicators(BUN,CREA,ALT,blood routine,electrocardiogram,fasting blood glucose).Results: A total of 60 patients with IMN were selected for this study.No patients with shedding or rejection were included.(1)TCM Syndrome Scores: After 3 months of treatment,the total effective rate was 93.33% in the experimental group and 66.67% in the control group,with significant differences(P<0.01).The total scores of TCM syndromes in the two groups were compared before and after treatment,and P<0.05 after treatment compared with before treatment;P<0.01 between groups.The test group was superior to the control group.In the two groups of patients with TCM syndromes after single score treatment,the experimental group was better than the control group in edema,poor appetite,poorappetite,aversion to cold extremities,abdominal distention,and urination,and there was a significant difference(P<0.05).There was no significant difference in stool weight(P>0.05).Within the group comparison,the experimental group and the control group were superior to those before treatment(P<0.01)in terms of edema,poor appetite,poor aversion to cold,abdominal distention,loose stools,and urine unfavorable treatment.(2)Laboratory Index: After treatment,patients in the test group and control group were compared with 24h-UTP,CHOL,and TG.All patients were significantly lower before treatment(P<0.05),and 24h-UTP,CHOL,and TG in the experimental group were lower than those in the control group after treatment(P<0.05);ALB in both groups was significantly higher than before treatment(P<0.05),in which the test group was significantly higher than the control group after treatment(P<0.05).(3)Safety index: There was no significant difference in BUN,CREA and ALT between the two groups before and after treatment(P>0.05).There was no significant difference in BUN,CREA and ALT after treatment between the two groups(P>0.05).No statistical significance.There was no significant change in liver function and renal function before and after treatment in the two groups.There was no significant change in electrocardiogram before and after treatment in both groups.There was no abnormal blood routine and fasting blood glucose did not increase.Conclusion: The clinical effect of Jianpi Bushen decoction combined with prednisone acetate and cyclophosphamide in the treatment of stage I and II idiopathic membranous nephropathy(spleen-kidney yang deficiency)is superior to that of prednisone acetate and cyclophosphamide alone,can significantly improve the patient's clinical symptoms of TCM,increase the level of ALB patients,and reduce the 24h-UTP,CHOL,TG levels,it is worth further promotion and research.
