| Objective Combined detection of serum PCT,whole blood WBC,explore the sputum PCT of ROC curve in the diagnostic value of community-acquired pneumonia of children,to guide clinical rational diagnostic methods,CAP etiology,severity and prognosis for children make scientific judgment.Methods On July 1,2016-June 30,2017,huizhou city,guangdong province,the third people’s hospital of pediatrics area of CAP of 86 children under the age of five years as the research object,according to the CAP infection in children with type,the condition of disease severity and prognosis of children group,compared to bacterial infection and bacterial infection group,mild condition and severe illness group children with no improvement,the improvement group and disease group serum PCT level and whole blood and sputum WBC level,computing each detection sensitivity index for the diagnosis of children CAP,specific degrees,false positive rate(1-specific degrees),ROC curve drawing,The area value(AUC)under the ROC curve was calculated,and AUC values of the three detection indicators were evaluated,and the diagnostic effect of each test index in the CAP of children was evaluated.Results(1)The levels of serum and sputum PCT level and total blood WBC count of the children with bacterial CAP were higher than that of non-bacterial children,and the difference was statistically significant(P < 0.05).(2)The levels of serum and sputum PCT and total blood WBC count in the children with severe CAP were higher than those with mild children,and the difference was significant and statistically significant(P < 0.05).(3)Those who improve and improve group serum PCT,sputum PCT,whole blood WBC level before treatment were higher,children three indicators to improve group were significantly lower after treatment,and no children each index levels improve group were no obvious changes,the differences between the two was statistically significant(P < 0.05).Spearman correlation analysis showed that both serum and sputum PCT level and total blood WBC were negatively correlated with the prognosis of children CAP(P < 0.05),and the relationship between PCT and prognosis was strongest among them.(4)the diagnostic sensitivity of sputum PCT,serum PCT,and whole blood WBC was 93.02%,86.05% and 75.58% respectively,and the difference of diagnostic sensitivity of the three indicators was statistically significant(P < 0.05).Specific degree 91.86%,56.98% and 60.47%,respectively,sputum PCT is significantly higher than whole blood WBC and serum PCT,compare the difference was statistically significant(P < 0.05),whole blood WBC and there was no statistically significant difference compared with the serum PCT in the diagnosis of specific degrees,(P > 0.05).Based on PCT,serum PCT,whole blood sputum ROC curve drawing,specific degree of sensitivity of the WBC and study analysis,including sputum AUC 0.996 PCT index of ROC curve,AUC 0.837 serum PCT index of ROC curve,AUC 0.690 serum index of the WBC ROC curve,including sputum PCT in children CAP diagnosis efficiency is highest,followed by serum PCT,finally the serum WBC.Conclusions CAP with sputum PCT,serum PCT,whole blood WBC level will rise,but higher than children with bacterial infection is not obvious,sputum PCT increased most obviously,especially in children with bacterial infection,severe illness in children with elevated levels of each index is more apparent.After treatment,the indexes of children with no improvement group were significantly higher than those in the improvement group.The diagnostic results of PCT,serum PCT and WBC were evaluated with ROC curve,and the diagnosis effect of sputum PCT was significantly better than that of serum PCT and whole blood WBC,which was more diagnostic value. |
