Background and objective:Gastric cancer is a common malignant tumor of the digestive tract.Its incidence and mortality are high,especially in developing countries.In recent years,the relationship between inflammation and tumor has attracted more and more researchers' attention.Researches on neutrophil-to-lymphocyte ratio(NLR)and platelet-to-lymphocyte ratio(PLR)in various cancers are also emerging in endlessly.However,the relationship between NLR,PLR and gastric cancer is not clear,there are different points of view.Therefore,this study aims to investigate the relationship between the level of NLR and PLR before surgery and the clinical pathological features and prognosis of patients with gastric cancer,and to clarify its value in predicting the prognosis of patients with gastric cancer.Methods:The clinical data of patients who met the inclusion criteria were collected retrospectively from January 2011 to December 2014 at the hospital of Yan'an University Affiliated Hospital.The patients were admitted to the Yan'an University Affiliated Hospital with pathology confirmed gastric adenocarcinoma.The data concludes gender,age,neutrophil,lymphocyte and platelet count within 1 week,tumor marker CEA,tumor location,tumor size,pathological grade,depth of tumor infiltration,lymph node metastasis,TNM staging.A total of 111 cases were included in the study.Patients were grouped according to their general characteristics and postoperative pathological results,NLR and PLR levels were compared in different grouped cases,and correlations between NLR,PLR and patients' survival time were analyzed.Results:The preoperative NLR level was correlated with the gender,tumor marker CEA,depth of tumor infiltration,lymph node metastasis and TNM stage(P<0.05).The preoperative PLR level was correlated with the gender,tumor marker CEA,Lymph node metastasis and TNM stage(P<0.05).Both of them were not related to the patient's age,tumor location,tumor size and differentiation degree,and the PLR was not related to the depth of tumor infiltration(P>0.05).Spearman rank correlation analysis showed that NLR(rs=-0.424,P=0.000)and PLR(rs=-0.366,P=0.000)were related to the survival time of patients with gastric adenocarcinoma.Multivariate analysis suggested that NLR(P=0.033),lymph node metastasis(P=0.010)and TNM(P=0.000)are independent prognostic factors of gastric adenocarcinoma survival.Conclusion:1.The level of NLR in peripheral blood of patients with gastric adenocarcinoma before surgery was related to their gender,tumor markers CEA,depth of tumor invasion,lymph node metastasis and tumor TNM staging.2.The level of PLR in peripheral blood of patients with gastric adenocarcinoma before surgery was related to their gender,tumor markers CEA,lymph node metastasis and tumor TNM staging.3.The increase of NLR and PLR values was negatively correlated with the survival time of patients with gastric adenocarcinoma.NLR was an independent predictor of survival time of patients,suggesting that it may be a simple and effective marker for poor prognosis of gastric adenocarcinoma patients. |
