Objective:1.To investigate the effects of T follicular helper and T regulatory cells on patients with thymoma and myasthenia gravis with thymoma,and healthy control group peripheral blood levels.2.To analyze the relationship between Tfh and Tfr cells and thymoma and MG,and to explore its significance in the pathogenesis of thymoma with MG.3.By detecting and analyzing the expression of Tfh and Tfr cells after co-culture of thymoma cells and normal human peripheral blood lymphocytes,the effect of thymoma cells on the differentiation of peripheral blood lymphocytes in normal people was explored.Method:1.Eighty two patients with thymoma and myasthenia gravis patients selected from Tianjin Medical University General Hospital from April 2016 to October 2017 as the experimental group.Another 20 patients with similar age,suffering from healthy volunteers as the control group of healthy subjects.Respectively included in the study of patients with peripheral fresh venous blood 3ml,into the refrigerator at 4?preservation.Peripheral blood mononuclear cells(Peripheral blood mononuclear cells,PBMCs)were isolated and stimulated by the peripheral venous blood lymphocytes of patients under study,and then labeled with antibodies.Finally,flow cytometry was used to detect Tfh and Tfr cells in simple thymoma and thymoma with MG and healthy control group of human peripheral blood levels.2.Thyroid tissue from 16 inpatients with thymoma was included in the inpatients as Thy0517 cells in the co-culture group.Sixteen healthy volunteers with age and gender matched above thymoma were selected or randomly selected peripheral blood from hospitalized patients with pneumothorax was used as healthy control group.Thy0517 cells and normal human peripheral blood T lymphocytes were cocultured by cell co-culture technique.Normal human peripheral blood T lymphocytes as a healthy control group.The expression of Tfh and Tfr cells in healthy control group and co-culture group were detected by flow cytometry and analyzed statistically.The influence of thymoma cells on the differentiation of peripheral blood lymphocytes in normal people was analyzed.Result:1.The percentages of Tfh cells in CD4~+T cells in MG patients with thymoma were significantly higher than those in other two groups,the percentage of Tfr cells in CD4~+T cells was significantly lower than the other two groups(P<0.05).The expression of Tfh in thymoma was higher than that in healthy controls(P<0.05).However,there was no significant difference in the expression of Tfr between the two groups(P>0.05).The expression of Tfh and Tfr cells in peripheral blood of patients with different pathological types of thymoma showed no significant difference,the difference was not statistically significant(P>0.05).There was no significant difference in the expression of Tfh and Tfr in peripheral blood of patients with different MGFA subtype MG,the difference was not statistically significant(P>0.05).The proportion of Tfh cells in CD4~+T cells in GMG patients was higher than that in OMG patients.The proportion of Tfr cells in CD4~+T cells was lower than that in OMG patients(P<0.05).Revealed the heterogeneity in the immune mechanisms of OMG and GMG.2.There was no significant difference in the expression level of Tfh between the monocyte group and the T lymphocyte group(P>0.05).The proportion of Tfh cells in CD4+T cells in the co-culture group was higher than that in the monocyte group and T lymphocyte stimulation group(P<0.05);Tfr cells in the three groups no significant difference in the expression level,the difference was not statistically significant(P>0.05).Conclusion:1.Thymoma indeed associated with the pathogenesis of MG;thymoma with MG patients Tfh cells showed an upward trend,Tfr cells showed a downward trend,the two cells may be involved in the pathogenesis of MG.2.Thymoma cells may affect the differentiation of Tfh and Tfr cells in peripheral blood.Cause Tfh cell expression increased,Tfr cell expression decreased.Thymoma may be related to the pathogenesis of MG.
