objectives :①To study the change of CD4~+CD25~+ T Regulatory Cells and CD95 in peripheral blood lymphocyte of myasthenia gravis(MG) patients,investigate the relation between CD4~+CD25~+ T Regulatory Cells and CD95 and the pathogenesis of myasthenia gravis.②To investigate the relation between the CD4~+CD25~+ T Regulatory Cells and CD95 and the severity of MG, and approach the immune maker in the early stage of MG patients; and provide a therapeutic basis in treatment of myasthenia gravis.③To approach mechanism of abnormal CD4~+CD25~+ T Regulatory Cells by the study of apoptosis,to study the relation between CD4~+CD25~+ T Regulatory Cells and CD95,as well as mechanism of immunological tolerance in MG patients .④To evaluate the clinical significance of CD4~+CD25~+ T Regulatory Cells and CD95 in the curative effect and prognosis of MG patients.Methods : 67 cases of MG were selected as study group,all from Department of Neurology and Chest surgery in Shan Dong provincial Hospital . According to the scale of absolutely clinical score, the MG patients were divided into two groups.the patient whose score were more than 8 defined as severe group, the others whose score were equal or less than 8 difined as mild group.Meanwhile 31 cases of subjects without immune disease and immunotherapy were selected as the control group. With tetrachromatism flow cytometry ,CD4~+CD25~+ T Regulatory Cells and CD95 were detected in peripheral blood lymphocyte of 67 cases of MG patients and 31 cases of health control, 30 cases of MG patients were retested after treatment.Results :①The ratio of expression of CD4~+CD25~+Tr was 7.38±1.21 in MG patients ,and lower than the healthy controls 7.95±1.27(P=0.0353 );The ratio of CD95 was 30.90±3.32 in MG patients ,and significantly lower than the healthy controls 37.28±3.58 (P=0.000)②The expression of CD4~+CD25~+Tr in mild MG patients was higher than that in healthy controls (P=0.001) ;the ratio of CD4~+CD25~+Tr in severe MG patients was lower than that in healthy controls (P=0.0000) .③The expression of CD95 in mild MG patients was higher than that in healthy controls (P=0.1625) but there was not significant difference between them;the ratio of CD95 of severe MG patients was lower than that in healthy controls (P=0.0000) .④CD95 was highly expressed on the surface of CD4~+CD25~+Tr (p<0.05) ;The expression of CD95 in MG patients was lower than the healthy controls (p<0.05) .⑤After three months with cortical hormone therapy ,the expression of CD4~+CD25~+Tr in MG patients was higher than before (7.52±1.32, 9.73±1.46) (P=0.0023) ;but the expression of CD95 in MG patients were not found significant difference after immunotherapy half a year (30.52±3.31,31.73±3.45) (P=0.4340) .⑥After thymectomy half a year, the expression of CD4~+CD25~+Tr in MG patients was significantly higher than before (7.43±1.12, 9.15±1.36) P=0.0064) ;but the expression of CD95 was not found significant difference after thymectomy (32.51±2.37,31.28±2.21) (P=0.2456) .Conclusions:①I t is found that there exists abnormal expression of CD4~+CD25~+ T Regulatory Cells and CD95 in peripheral blood lymphocyte of MG patients; the decrease of expression of CD95 is associated with damaged mechanism of immunological tolerance of MG.②The results indicate that the ratios of CD4~+CD25~+ T Regulatory Cells and CD95 are connected with the severity of MG patients, in the mild MG patients CD4~+CD25~+ T Regulatory Cells and CD95 express highly, therefore ,they can be taken as useful marker for abnormal immune state of MG.③CD95 is highly expressed on the surface of CD4~+CD25~+Tr.④CD4~+CD25~+ T Regulatory Cells and CD95 are beneficial for evaluating the therapeutic effect and prognosis of MG,so they have clinical significance during the treatment of MG. Objective : To investigate the level and the clinical role of the Interferon-αantibodies (IFN-αAb ) in myasthenia gravis (MG ) patients , To study the relation between IFN-αAb and abnormal thymus in MG patients.Methods : IFN-αAb were assayed in 89 cases of MG patients(MG group) with enzyme-linked immunosorbent assay (ELISA), 31 case of normal controls were selected .The level of IFN-αAb were compared among MG patients with different abnormall occurrence of thymus(thymoma, thymic hyperplasia , thymic atrophy ),normal thymus;as well as with different age MG patients..Results :①The positive rate of IFN-αAb was 19% in MG group , not higher than the normal controls group (P =0.0658).②Male patients and 40y older patients were prevalent in thymoma-associated MG group (the percentages were 86% and 82%). The positive rate of IFN-αAb was 64% in thymoma-associated MG group , also higher than non- thymoma MG group (P<0.01),5% in thymic hyperplasia with MG patients,2% in thymic atrophy with MG patients,and 2% in normal thymus with MG patients.The sensitiveness of IFN-αAb in thymoma-associated MG group was 64% (14/22) and the specificity was 96% (94/98) .③The positive rate of IFN-αAb was 35% in age of 41~72y groups,and significantly higher than age of 17~40y group (P<0.05) .④There was no significant correlation between IFN-αAb and the severity of the clinical symptom.Conclusion : The expression of IFN-αAb is significantly higher in thymoma-associated MG group and age of 40y older patients.The level of it is important to discriminate thymoma-associated MG. therefore it is useful for treatemt of MG.
