| Objective : To evaluate and compare the curative effect and safety of fractional microneedle radiofrequency device and non-ablative laser procedure in the treatment of striae distensae.Methods:This study recruited 15 volunteers(skin type Fitzpatrick III-IV)with large area of symmetrical stretch marks lesions on abdomen,and have cognitive competence and voluntary participation in this study.Exclusion criteria: pregnant and lactating women,patients with systemic disease,scar diathesis,skin diseases or skin cancer,and patients with mental disorders or contraindications to laser treatment.Informed consent and photo usage agreement were obtained from every participants and possible risks and benefits of the treatment is fully informed.Before the first treatment,participants’ general condition was recorded to establish their personal files.Abdomen area divided into A B side by the midline which was divided randomly through tossing coin.Treatment side A received fractional microneedle radiofrequency therapy while side B side received non-ablative laser therapy.Fractional microneedle radiofrequency side use bipolar RF with energy from 16 w to 18 w,microneedle parameter Depth for 2-3mm,and the conduction time for 600 ms.Non-ablative laser side comprised two passes over the SD with different setting.First pass using rectangular pattern aimed on each stria and parallel to it’s long axis,using 300 microspots/cm 2,energy ranged 40 – 44 mJ/cm 2.And then treated whole area with hexagonal shape for150 microspots/cm 2 and 50 – 55 mJ/cm 2.Participant received 3 sessions of treatment with a interval of 5 to 6 weeks.Before treatment,the topical compound lidocaine cream was applied for over 40 minutes to anesthesia.After that,we used normal saline to wipe clean,alcohol to disinfect and make sure the area had been dried before our treatment.After treatment,applied desonide cream and epidermal growth factor gel to the area.Evaluation of therapeutic effect: Photographs for evaluation were taken with digital camera in the hospital photography room before each treatment and 3 months after the last treatment,with the measure of skin melanin index(MI).Three months after the last treatment,the doctor and the participants evaluated the curative effect on both sides of the treatment area respectively。Pain was assessed by participants on both sides after treatment.The adverse reactions should be cautiously recorded by participants。 Results:Doctors’ evaluation: At 3 months post-final treatment,the improvement rate of the SD on the side of fractional microneedle radiofrequency(group A)was 78.57%,and the improvement rate was 64.29% on the side of 1565 nm non-ablative laser(group B),and the difference between the two groups was insignificant(P>0.05).In terms of the improvement of relaxation,the subjective effective rate of doctors in group A and group B was both 28.57%.There was a certain difference in the participants’ satisfaction with both sides,with A side satisfaction score of 2.14 and the satisfaction rate of 50.00%.B side satisfaction score was 2.5,and the satisfaction rate was 71.43%,which was not statistically significant to the another side.Both sides of the treatment area recovered well,no other long-term adverse reactions occurred.Both group A and group B found a raising MI value,but there was no statistically significant difference between the two groups.The variation of MI in group A first was statistically significant(P = 0.0075).group B changes had no statistical significance.Conclusion:Fractional microneedle radiofrequency device and 1565 nm non-ablative laser both have very good efficacy in the treatment of striea distensae with no adverse reactions at 3 months post-final treatment.Fractional microneedle radiofrequency shows better curative effect than 1565 nm non-ablative laser device.In consideration of the pain,participants were more satisfied with the latter. |
