Objective:To compare the efficacy and safety of 2940nm ablative fractional laser and 1565nm non-ablative fractional laser in the treatment of photoaged skin.Methods:After excluding contraindications among the volunteers,eleven photoaged Chinese women that range in age from 40 to 60 and have Fitzpatrick skin types ? and IV were recruited for this study.All the patients were informed of risks,benefits,postoperative complications and signed the informed consent.One side of the patient's face was randomly selected to receive the treatment of using 2940nm laser,and the other side was treated with 1565nm laser.Each patient received three treatments at one-month intervals.The photoaged skin were evaluated prior to treatment and 3 months after the last treatment.Took photos with VISIA and completed the clinical evaluation.PRIMOS 3D was used to measure the Ra value and Rz value around the lateral canthus,and retain the skin color height image,three-dimensional skin image and three-dimensional skin plaster sample.The skin melanin index(MI)and erythema index(EI)of 10 different parts(1cm above the median of eyebrow,1cm outside of the lateral canthus,on the top of the cheekbones,1cm outside of the angulus oris,in the middle of mandibular and angulus oris)of face were measured by Mexameter MX18.Three months after the last treatment,the self-evaluation method was used to evaluate the improvement of facial skin.Subjects evaluated the pain after the first treatment and described other adverse reactions occurred during the whole experiment.SPSS software was used for analysis and processing,t test for normal distribution data and rank-sum test for non-normal distribution data,P<0.05 was considered to be statistically significant.Results:1.Self-evaluation of the subjects:Three months after the last treatment,all subjects thought no obvious difference between the two kinds of laser treatment,ten percent of the subjects were great satisfied with the changes in skin texture after the treatment of the two kinds of laser,fifty percent of the subjects were satisfied with the changes in skin texture,thirty percent of the subjects felt general about the changes in skin texture,ten percent of the subjects were not quite satisfied.2.Clinical evaluation:the scores of all patients for crow's feet after the two laser treatments were lower than before(P<0.05),there was no significant difference between the two laser treatments(P>0.05).3.Measurement results obtained by PRIMOS 3D:There was no significant difference both in Ra and Rz value around the lateral canthus after the two laser treatment(P>0.05).4.Adverse reactions:1)Eighty percent of the subjects thought that the pain caused by 2940nm laser was worse than 1565nm laser after the treatment.One of the subjects showed obvious pigmentation after the treatment.No blisters,hypochromia,scarring and infections occurred.2)Measurement results of Mexameter MX18:The MI value was increased 1cm outside of the lateral canthus and 1cm outside of the spat after 2940nm laser treatment(P<0.05).The MI value was increased 1cm outside of the lateral canthus,on the top of the cheekbones and 1cm outside of the spat after 1565nm laser treatment(P<0.05).There was no significant difference in MI values after the two laser treatment(P>0.05).There was no significant change in El values after 2940nm laser treatment(P>0.05).The El value was increased 1cm outside of the lateral canthus,on the top of the cheekbones and 1cm outside of the spat after 1565nm laser treatment(P<0.05).There was no significant difference in El values after the two laser treatment(P>0.05).Conclusion:Under the equipments and treatment parameters of this study,both 2940nm laser and 1565nm laser are effective in treatment of changes in texture of photoaged facial skin,and there was no significant difference in effect.2940nm laser had a lower risk of producing erythema than 1565nm laser in the treatment of photoaged skin.2940nm laser generated a greater discomfort than 1565nm laser after the treatment.
