Study On The Quality Of Life And The Influencing Factors In Type 2 Diabetes Mellitus Accompanying With Depression Among Inpatients

Objective: It’s aim to study inpatients that were in T2 DM with depression and the relationship between depression and quality of life and the influencing factors on depression.Methods: From October 2016 to September 2017,a total of 376 inpatients with T2 DM were included from department of endocrinology in Shaanxi Provincial People’s Hospital.Patients’ health questionnaire-9(PHQ-9)and World Health Organization Quality of Life-BREF(WHOQOL-BREF)were used to assess depression status and quality of life.According to the PHQ-9 score the patients were divided into depression group(PHQ-9≥5)(n=195)and non-depression group(PHQ-9<5)(n=181)as study group and control group,respectively.The two groups of patients were studied in the four areas of physical health,psychological health,social relationships and environment.Meanwhile,the influencing factors on depression were analyzed in demographic data,general clinical data,numbers and type of chronic complications,and accompanying disease.Results: 1.The prevalence of depression in hospitalized T2 DM patients was 51.86%(n=195),including 118(31.39%)mild depression cases,46(12.23%)moderate depression cases,24(6.38%)moderately severe depression cases and 7(1.86%)severe depression cases.2.The quality of life scores in study group were significantly lower than control group in four fields of physical health,psychological health,social relationships and environment(all P<0.001),meanwhile the severity of depression and the quality of life score were negatively related(r =-0.475,-0.369,-0.168,-0.244)(all P<0.05).3.Univariate has shown that age,occupation,marital status,education level,course of disease,glycosylated hemoglobin A1c(Hb A1c),diabetic retinal neuropathy and diabetic peripheral neuropathy were related with depression(all P<0.05),however,while gender,smoking history,body mass index(BMI),waist circumference,the number of complications,diabetic nephropathy and the stage,diabetic lower-extremity arterial disease and hypertension had no relationship on depression(all P>0.05).Multivariate logistic regression analysis has shown that 60~69 years old(OR=0.352,95%CI:0.161~0.771,P=0.009),freelancer(OR=0.263,95%CI: 0.087~0.794,P=0.018)and higher education level(junior: OR=0.412,95%CI: 0.176~0.964,P=0.041;senior: OR=0.293,95%CI: 0.123 ~ 0.698,P=0.006;junior college: OR=0.060,95%CI: 0.022 ~ 0.163,P=0.000;bachelor’s degree and above: OR=0.253,95%CI: 0.099~0.649,P=0.004)all were protective factors on depression.Peasant(OR=3.609,95%CI:1.134 ~ 11.488,P=0.030),non-marital status(OR=2.796,95%CI:1.297~6.030,P=0.009),course of disease on <1 years or ≥10 years(<1years:OR=3.147,95%CI:1.356~7.301,P=0.008;≥10 years: OR=2.048,95%CI:1.150~3.646,P=0.015),diabetic retinopathy(OR=2.255,95%CI:1.201 ~ 4.233,P=0.011)and diabetic peripheral neuropathy(OR=3.131,95%CI:1.819~5.390,P<0.001)were risk factors on depression in patients with T2 DM.Conclusions: The prevalence of depression in hospitalized T2 DM patients was high and the quality of life in patients with depression was poor,meanwhile,there was negative relationship between the degree of depression and the quality of life.60~69 years old,self-employed,and higher education level might be independently protective factors,however,peasant,non-marital status,course of disease on <1 years or ≥10 years,diabetic retinopathy and diabetic peripheral neuropathy might be independently risky factors of T2 DM inpatients with depression.
