| Objective:Breast cancer has become one of the most common malignant tumors among women in the world,which has seriously endangered women’s lives and health.It has increasingly attracted worldwide attention.According to epidemiological statistics,the global incidence and mortality of breast cancer in women have significant geographical distribution differences.Overall,the results in North America is significantly higher than that in East Asia and Central Africa.In recent years,the incidence and mortality of breast cancer in China have shown a significant upward trend in both urban and rural areas,and the rate of increase in rural areas is even greater.With the deep understanding of breast cancer,looking for biomarkers that can be used for early diagnosis,treatment and prognosis,has become a hot spot in the research.At home and abroad,the relationship between VEGF and breast cancer is mostly investigated by immunohistochemistry,and most of the conclusions are consistent,suggesting that VEGF is highly expressed in breast cancer tissues,which is correlated with clinical stage,histological grade and lymph node metastasis.While the detection of VEGF level in serum is relatively small,and the conclusions are inconsistent.In addition,there is strong evidence that HER-2 is important in the development and progression of breast cancer.However,in recent years,there has been little research at home and abroad on whether there is a relationship between VEGF and HER-2 in breast cancer.With the intensive study of the combination therapy targeting VEGF and HER-2,a solid theoretical basis is needed to confirm the relationship between the two.This article examines the serum VEGF levels which included in breast cancer patients and benign breast disease patients,comparing the levels in two groups.And explore the relationship between serum VEGF levels and clinical factors in breast cancer and correlation with HER-2 expression in cancer tissues.The results provide a theoretical basis for the early diagnosis,targeted therapy and prognosis evaluation of breast cancer patients.Methods:1.Clinical data1.1 Experimental group: A total of 146 cases of breast cancer patients treated in our hospital from September 2016 to June 2017 were selected as experimental group.Inclusion criteria are as follows:(1)All patients were femal(2)The patients who had clear diagnosis of breast cancer(3)The patients who underwent breast cancer surgery(4)None of the patients had chemoradiation,endocrine therapy and targeted therapy before surgery(5)Patients with complete clinical and pathological data(6)Patients completed serum VEGF detection before operation(7)All patients who participated in the study signed informed consent(8)The experiment was approved by the Hospital Ethics Committee.Exclusion criteria:(1)Patients with a history of other oncological diseases outside breast cancer(2)Immunohistochemistry with HER-2 as 2+ but no FISH detection(3)Patients with a history of platelet-associated hematologic diseases.1.2 Control group: Select 30 cases of benign breast disease patients which in the same period.2.Method2.1 Serum VEGF detection methods: Enzyme-linked immunosorbent assay ELISA was used to detect serum VEGF level.2.2 Detection of HER-2: The expression of HER-2 in breast cancer tissues was detected by immunohistochemistry.2.3 Relationship between serum VEGF and breast cancer:The serum VEGF levels of the test group and the control group were examined,and the relationship between serum VEGF levels and breast cancer was studied.2.4 The relationship between serum VEGF levels and clinical factors: The relationship between serum VEGF levels and age,tumor size,lymph node metastasis,histological grade was studied in the experimental group.The ages were divided into <50-year-old group and ≥50-year-old group.The tumor size was divided into ≤2cm group,2~5cm group,and >5cm group.Lymph node metastasis was divided into 2 groups according to lymph node metastasis.Histological grade was divided into Level I group,Level II group and Level III group.Age,tumor size,lymph node metastasis and histological grade were collected directly from clinical cases and pathological reports.Tumor size,lymph node metastasis,and histological grade were all based on postoperative pathological results.2.5 The relationship between serum VEGF levels and HER-2 expression in tissues: The levels of serum VEGF and HER-2 expression in tissues were observed in the experimental group and the relationship between the two was studied.3.Statistical Methods: The relevant data of this study was processed by SPSS24.0 software.Non-normal distribution of measurement data using the median M,with P<0.05 for the difference was statistically significant.Results:1.The levels of serum VEGF in breast cancer and benign breast disease patients: The median of serum VEGF level in breast cancer patients was 51.46pg/ml,while it was 34.81pg/ml in breast benign disease patients.The difference was statistically significant(P=0.002<0.05).2.The relationship between serum VEGF levels and HER-2 expression in breast cancer patients: The median of serum VEGF level was 56.04pg/ml in HER-2 positive breast cancer patients,which was significantly higher than that in HER-2 negative patients(46.92pg/ml),the difference was statistically significant(P=0.003<0.05).3.The relationship between serum VEGF and tumor size in breast cancer patients: The median of serum VEGF level was 60.1pg/ml in patients with tumor diameter> 5cm,higher than that in patients with diameter 2-5cm(56.42pg/ml)and diameter≤2cm(45.96pg/ml),the difference between the three was statistically significant(P <0.05).4.The relationship between serum VEGF and histological grade in breast cancer patients: The median serum VEGF level was different in different histological grades(P=0.006<0.01).The median level was 64.97pg/ml in histological grade III,which was significantly higher than histological grade I(36.75pg/ml)(P=0.034<0.01)and histological grade II(50.79pg/ml)(P=0.003<0.01),the difference was statistically significant.However,the median level of VEGF was not statistically significant between histological grades I and histological grade II(P>0.05).5.The relationship of serum VEGF and lymph node metastasis in breast cancer patients: The median of serum VEGF level in patients with lymph node metastasis was 55.91pg/ml,which was higher than that in patients without lymph node metastasis(50.59pg/ml)(P=0.030<0.05).6.The relationship of serum VEGF and age in breast cancer patients: In the age category,the median of its level was 51.90 pg/ml in patients ≥50 years of age and 51.13 pg/ml in patients <50 years of age,and there is no difference statistical significance(P>0.05).Conclusion:1.Serum VEGF level in patients with breast cancer was significantly higher than in benign breast disease patients,which can be concluded that serum VEGF and breast cancer is closely related to the occurrence and development.2.The level of serum VEGF is related to the status of HER-2.The level of serum VEGF was increased in HER-2 positive patients,while it was lower in HER-2 negative patients.This result may provide a theoretical basis for a combination therapy targeting VEGF and HER-2.3.The level of serum VEGF was related to the size of tumor,histological grade and lymph node metastasis in breast cancer patients,indicating that it can be used as an important index to evaluate the prognosis of breast cancer. |
