Objective:To discuss the complications and curative effect analysis about CO2 laser therapy on early glottic cancer, To provide reference for clinical treatment.Methods:From January 2008 to October 2014, Retrospective analysis of clinical data about 61 patients (Tis, T1a, T1b and T2 was 5,40,9 and 7 cases) who diagnosised early glottis cancer, were underwent CO2 laser therapy. To disscuss the relationship of postoperative complications, local recurrence rate and T stage, surgical classification and pre joint involvement. To analysis and summary the indications, complications and curative effect about CO2 laser therapy on early glottic cancer.Results:The follow-up period ranged from 16 to 95 months, with a median follow-up time of 41 months. Of 61 cases 58 cases survived, the 3-year survival rate was 95.8%, the 5-year survival rate was 92.7%.21 patients had complications after operative in 2-4months, incidence rate was 32.8%(20/61): Vocal cords adhesion 9 cases, granuloma 10 cases, stenosis 1 case. The incidence of complications of Tis, T1a, T1b, T2 cases of glottic cancer were 20%,22.5%,55.6% and 71.4%, respectively, with significant difference (P=0.022). Incidence of complications in the patients underwent type II, III, IV, V cordentomy were 11.1%,15.4%,23.8%, and 66.7% respectively, with significant difference (P=0.002). Incidence of complications of glottis carcinoma with and without anterior commissur indision were 42.9% and 27.5%, with no difference between the two groups (P=0.225). The local recurrence rate of Tis, Tla, T1b, T2 cases were 0,12.5%,33.3%, and 57.1% respectively, with significant difference(P=0.019). The local recurrence rate of glottis carcinoma with and without anterior commissur indision were 33.3% and 12.5% respectively, with significant difference (P=0.022).3 cases had total laryngectomy after recrudesced, The laryngeal function preservation rate were 95.1%(58/61)Conclusions:1. Be careful to evaluate of tumor range before operation, strict grasp of laser surgery indications can reduce the surgical complications and recurrence rate, Improve the quality of patients life.2. The incidence of complications of CO2 laser therapy on early glottic cancer were positively correlated with tumor stage and types of laser surgery, the higher the stage, the higher the incidence of complications. The relationship between complications and whether the former joint involvement is still unknown.3. The recurrence rate of CO2 laser therapy on early glottic cancer were positively correlated with tumor stage. And the recurrence rate of patients with Anterior commissure invision was higher than that without Anterior commissure invision.
