ObjectivesTo evaluate the sensitivity,accuracy and specificity of Ultrasonography with the second version of ultrasonic BI-RADS(Promulgated in 2013)and to evaluate the application value of those sonographic features enumerated in the second version of ultrasonic BI-RADS in the diagnosis of malignant breast masses.MethodsThe static images of the 2860 masses and the relative clinical materials collected were retrospectively analyzed and classified with the guidance of the second version of ultrasonic BI-RADS.Based on golden standard(pathological results),the sensitivity,specificity and accuracy were calculated by ROC curve.Fifteen recorded sonographic descriptors and two clinic materials were analyzed using univariate analysis firstly,and then the descriptors with statistical significance were analyzed by multifactor logistic regression analysis.ResultsAmong the 2860 breast masses,88.4% was benign and 11.6% was malignant.The second version of ultrasonic BI-RADS showed high sensitivity,NPV,accuracy,specificity;and the AUC was 0.948.Changes of duct and Cooper’s ligament had no statistical significance in univariate analysis.Fourteen variables were screened out from fifteen sonographic descriptors by multifactor logistic regression analysis.ConclusionsThe second version of ultrasonic BI-RADS can be widely generalized in clinic with high diagnostic efficiency in assessing benign and malignant breast masses.And the meaningful characteristic variables of ultrasonic BI-RADS for the diagnosis of breast cancer can be screened out with logistic regression analysis,in which the shape,edge,calcification and blood flow status of the tumor are important ultrasonic variables,combined with patient age and axillary lymph node metastasis can guide clinicians for clear diagnosis and precise treatment.
