Objective:To investigated the main pathogenic factors of SSI among patients those who undergone cardiac open chest surgery(OCCS)and find out flora distribution of the SSI patients.Try to make effective measures to preventing post-operation SSI.Methods:327 consecutive patients undergoing cardiac open chest surgery were investigated in this research.Cases were grouped by whether the patients got a SSI after OCCS or not——SSI(67)vs NON—SSI(260).The baseline information of all cases were collected and analyzed respectively.Using SPSS to analyze the data.Results:There is a significant difference between two groups.The observing group had a higher percent of diabetes,COPD,RF,hypoproteinaemia,glucocorticoids taking history(P<0.05).Those patients with SSI often undergoing a longer operation time,a larger intraoperative hemorrhage(IH)and more susceptible to invasive mechanical ventilation(IMV).Further analysis of these factors,the observing group had a higher percent of hypoproteinaemia(OR 5.11(95%CI 1.69-15.47);P<0.05),longer operation time(OR 1.01(95%CI 1.00-1.02);P<0.05),glucocorticoids(OR 33.18(95%CI 5.85-188.12);P<0.05)and longer invasive mechanical ventilation(OR 1.20(95%CI 1.04-1.39);P<0.05).There are total 67 cases of culture specimens,fifty-seven cultures are positive,including 36 G~+,19 G~-,2fungus.The main G~+pathogens are S.epidermidis and S.aureus,acinetobacter baumannii and pseudomonas aeruginosa are main G~-.Conclusion:There is a number of factors can lead to SSI post—OCCS,in this research we find the main reasons are hypoproteinaemia,operation time,glucocorticoids and IMV.Those patients who complicated with above factors have a higher risk of SSI and usually spend more money on treatment,when confronted with this situation,practitioners should diagnose and treat them as soon as possible.There is a relative high rate of patients who were infected by Gram-negative bacteria,it indicates that G~-should be taken into our consideration when using antibiotics.Thoes groups with uncompleted immune system have a risk of fungal infection which should be taken into consideration by clinicians.
