| OBJECTIVE: Keratograph 5M ocular surface synthesis analyzer was used to observe the characteristics of ocular surface lesions in type 2 diabetes,and to explore the correlation between diabetes duration and blood glucose control stability and ocular surface damage.METHODS: From October 2017 to January 2018,70 patients were admitted to the ophthalmic clinic of the First Affiliated Hospital of Fujian Medical University.According to whether the type 2 diabetes diagnosed was divided into diabetes group and non-diabetic group,there were 36 cases in the diabetes group(72 eyes),including 18 males with 36 eyes(50%),18 females with 36 eyes(50%),an age range of 58.89±7.54,and non-diabetic patients with 34(68 eyes),including 16 males There were 18 females and the age range was 54.32±9.24.Case-control study of two groups of subjects:1.After the basic information acquisition and Ocular Surface Disease Index(OSDI)investigations were performed on both groups of patients,the tear height and non-invasive tear film rupture time(BUT)was measured with a Keratograph 5M ocular surface analyzer.Lipid layer observation,meibomian gland morphology and loss area,meibomian gland opening,conjunctival hyperemia index,corneal fluorescence staining and other data were collected,the collected data were analyzed using SPSS 24.0 software;2.The diabetes group Diabetes duration,glycosylated hemoglobin and ocular surface damage were correlated.RESULTS: Compared with the non-diabetic group,the diabetes group had significantly higher OSDI score than the non-diabetic group(P<0.05).A comprehensive examination of the subjects using the Keratograph 5M Eye Surface Analyzer revealed that: The height of tears,the time for the first tear film rupture and the average tear film rupture time were significantly lower in the diabetic group than in the nondiabetic group(P<0.05);The uneven distribution of lipid layer and the single color ratio were significantly higher than those in the non-diabetic group(P<0.05);Conjunctival hyperemia index,meibomian gland absent area score,meibomian gland opening obstruction rate,corneal fluorescence staining rate and scores were significantly higher than non-diabetic group(P<0.05).The above data has no significant correlation with diabetes duration.There was no significant correlation between tear height,redness index,lipid layer observation and glycosylated hemoglobin,while the first BUT,mean BUT and glycosylated hemoglobin were of moderate intensity negative correlation,OSDI score,dry eye grading,upper temporal bone gland analysis,and diarrhea.Plate gland analysis and meibomian gland scores,corneal fluorescence staining score and glycosylated hemoglobin were of medium intensity positive correlationConclusion: Compared with non-diabetic patients,symptoms of dry eye in diabetic patients were more pronounced and severe,tear height decreased,tear film rupture time and tear film rupture time were shortened for the first time,and lipid distribution was more uneven and the color was more uniform.As the area of deletion increased,the corneal fluorescence staining rate increased.The degree of diabetes damage to the ocular surface gradually increased with the increase of glycohemoglobin,but there was no significant correlation with the duration of diabetes.The Keratograph 5M Eye Surface Analysis Analyzer provides objective,non-invasive,comprehensive and repeatable observations of the ocular surface state,providing a basis for the observation,diagnosis,treatment,and curative effect evaluation of diabetic ocular surface damage. |
