Salvage Operation System Evaluation For The Treatment Of Local Recurrence And Regional Recurrence Laryngeal Cancer

Objective:A systematic comparison of the survival of local recurrent groups and regional recurrence groups after laryngeal carcinoma surgery.through meta analysis of the database retrieval literature for the survival rate of local recurrence and regional recurrence,we would Have a further understanding to the prognosis of laryngeal carcinoma recurrence and in different parts provide basis for evidence-based medicine.Methods:Through the retrieval of CBM,wanfang,CNKI,weipu,PubMed,OVID,Google,Springe,etc from inception to 2017-11 by the end of the month,recurrent laryngeal carcinoma included in the study as the research object,in line with the conditions,the using is RevMan5.3 software to extract data for Meta analysis.According to the heterogeneity test selection effect model,the classification system of two variables using local recurrent groups and regional recurrence groups of laryngeal carcinoma was the odds ratio(OR)and 95% confidence intervals(95%CI).in 5 years and 3 years survival rate as the index,survival comparison of local recurrence groups and regional recurrence groups and The "funnel plot" was made to observe its publication bias.Results:Finally,13 retrospective studies were included,including 2044 cases,561 eligible cases,including 287 cases in the local recurrence group,244 Cases in the regional recurrence group,and 30 cases in the rest group.Meta analysis results show that(1)the 5 year survival rate: a total of 9 articles,a total of 397 cases,including local recurrence in 218 cases,5 year survival of 111 cases,179 cases of regional recurrence group,5 years survival was statistically significant in 42 cases.the 5 year survival rate was statistically significant difference in local recurrence group and regional recurrence group(p=0.0003),the merger odds ratio and 95% confidence interval was 3.32(1.74,6.36);(2)the 3 year survival rate : a total of 7 articles in line with the conditions,a total of 246 cases,including local recurrence in 136 cases,3 year survival of 71 cases,110 cases of regional recurrence group,3 year survival of 40 cases and the 3 year survival rate was statistically significant difference in local recurrence group and regional recurrence group(P=0.02),the merger odds ratio and 95% confidence interval was 1.97(1.11,3.51).Conclusion:The 13 literatures included the related contents of the local recurrence group and the regional recurrence group.The research shows that the 3 and 5 year survival rate of the local recurrence group is higher than that of the regional recurrence group,indicating that the prognosis of local recurrent laryngeal cancer is relatively better.Due to the influence of the salvage operation,the first treatment and the classification of the stages,and the lack of the number of cases,the related conclusions should be further studied.
