Backgroud:Gastric cancer is a common malignancy in the world.Its morbidity and mortality are located at the second place in malignant tumors,which seriously threatens human health.Early gastric cancer can be treated with endoscopic treatment with 5-year survival rate exceeding 90%.Even after undergoing comprehensive treatment based on surgery,the postoperative recurrence rate of advanced gastric cancer is 40%-70%,and the 5-year survival rate is still lower than 30%.Although chemotherapy is the most important postoperative medical treatment,its toxic and side effects make it difficult for most patients to tolerate,and the extension of the overall survival of postoperative patients is limited.Traditional Chinese medicine has a long history and tradition in the treatment of patients with gastric cancer.It can improve symptoms,enhance quality of life,and prolong survival.Exploring the evaluation of the efficacy and safety of Chinese medicine in preventing or delaying the recurrence and metastasis of gastric cancer is of great significance for the comprehensive prevention and treatment of gastric cancer.Objective:This study intends to evaluate the effect of Fuzhengjiedu method on the recurrence,metastasis and quality of life of gastric cancer after the combination of clinical studies and literature reports.The purpose of this study was to investigate the effect of Fuzhengjiedu method in preventing gastric cancer recurrence and metastasis after intervention of gastric cancer patients and its influence on the quality of life of patients after gastric cancer surgery.It provides medical evidence for reducing the recurrence and metastasis rate of postoperative gastric cancer by TCM treatment,indicating the role of Chinese medicine in preventing and treating the recurrence and metastasis of postoperative gastric cancer patients.Method:With forward-looking research design,patients who meet the inclusion criteria are treated with righting and detoxifying Chinese medicine + symptomatic and supportive care.The cumulative treatment time for using traditional Chinese medicines is not less than 3 months,and other anti-tumor treatment methods may not be used in combination withChinese medicine.Follow up every 6 months,follow-up time is not less than 12 months,with QLQ-ST052(QLQ-C30 and QLQ-ST022)Quality of Life Scale.Statistical methods were used to calculate the recurrence and metastasis rates at 12 months,18 months,and 24 months,and whether the quality of life of patients before and after treatment was improved by self-analysis.Results:A total of 78 patients were enrolled in the study,78 of whom were observed after 12 months of follow-up,61 patients who completed 18 months of observation,and 43 patients who completed 24 months of observation.1.Basic information:Of the 78 patients in this study,the ratio of males and females was 3.88:1,and there were 55 patients aged 50-69 years old.In the pathological type,a total of 71 cases of adenocarcinomas accounted for the most(89.87%);the differentiation degree was mainly poorly differentiated in 46 cases(64.79%);the gastric antrum was the predilection site of the study;diffuse type in the Lauren classification In most cases,the stage of tumor is mainly in stage III,and the degree of tumor infiltration is mainly T3.In the degree of lymph node metastasis,the number of cases of N0,N1,N2,and N3a is similar.The surgical approach was based on traditional laparotomy and laparoscopic-assisted surgery;66 patients received chemotherapy,on cycles of 6;and 12 patients received only pure Chinese medicine in addition to surgery.2.Recurrence rate:The recurrence rates of 12 months(1 year),18 months(1.5 years),and 24 months(2 years)of postoperative gastric cancer patients in this study were 5.13%(4/78)and 13.11%(8/61)and 18.60%(8/43).The disease-free survival(PFS)ranged from 7.67 to 68.67 months and the median DFS was 22.17 months.3.Quality of life assessment:Through the analysis of the EORTC QLQ-ST052 Quality of Life Scale,patients in the 5 functional areas,18 symptom areas,and total health levels improved at each stage after treatment compared with before treatment.The total health level improved significantly.In addition to cognitive function areas,the improvements in the areas of physical function,role function,emotional function,and social function were statistically significant.Regarding symptoms,the improvement in fatigue,pain,constipation,nausea,vomiting,insomnia,and shortness of breath was statistically significant.Conclusuion:1.In this study,in 12 months(1 year),the cumulative recurrence and metastasis rate was 5.13%,18 months(1.5 years)cumulative recurrence metastasis rate was 13.11%,and 24 months(2 years)cumulative recurrence metastasis rate was 18.60%.Compared with previous reports,it can be shown that Fuzheng Jiedufang can reduce the recurrence and metastasis rate of gastric cancer patients to some extent.2.Through analysis of the EORTC QLQ-ST052 scale,it was demonstrated that Fuzhengjiedu method has the effect of relieving clinical symptoms(mainly by fatigue,pain,constipation,nausea and vomiting,insomnia and shortness of breath)and improving quality of life in the treatment of gastric cancer patients.
