| Objective:To understand the status of antihypertensive drug use and medication compliance in hospitalized patients in a hospital in qiqihar,and to explore the related influence factors of patients with high blood pressure medication adherence,in order to provides a theoretical basis for rational guidance of medication use in patients,improvement of medication compliance in patients with hypertension,and improvement of blood pressure control in patients with hypertension.Methods:427 patients with primary hypertension in the department of cardiovascular medicine of a hospital in qiqihar from January 2018 to May 2018 were included in this study.EpiData 3.1 was used for data entry,SPSS22.0 software was used for statistical analysis,and the patient’s general characteristics and drug types were compared for descriptive analysis.Chi-square test was used to analyze the inter-group comparison of counting data,and the significant variables in univariate analysis were incorporated into the Logistic regression model for multivariate analysis.P<0.05 was considered statistically significant difference.Results:1.A total of 427 subjects were enrolled in the study,157(36.8%)males and 270(63.2)females;the mean age was 65.85±11.44 years,among them,68.9%of patients over 60 years old;43.5%of patients took antihypertensive drugs for more than 5years.2.The study found that among the 6 types of antihypertensive drugs,the proportion ofβ-RB drugs was the highest,accounting for 39.7%of the number of drugs,followed by CA and compound preparations,accounting for 28.4%and 19.9%of the number of drugs respectively.A total of 56.4%of patients were treated with a combination therapy,of which two-drug regimen was most commonly used.The most commonly used two-drug regimen was CA+β-RB,accounting for 51.4%of all two-drug regimens.The most common among the triple regimens is DA+β-RB+ARB,which accounts for 60.0%of the triple drug users.The vast majority(92.0%)of patients taking quadruple medications used the CA+DA+β-RB+ARB regimen.3.26.0%of the patients took the medication according to the doctor’s instructions strictly.The average patient compliance score was(3.22±2.23),and the number of medication adherence scores above 6(medium or better compliance)was71,accounting for only 16.6%.4.The average score of hypertensive knowledge was(6.33±4.22),with 4.2%of those with knowledge of hypertension.Patients short of or with poor knowledge of hypertension accounted for 33.3%and 52.0%respectively.5.The study found that the patients with different levels of education(χ~2=13.977,P=0.003),diagnosed hypertension years(χ~2=8.284,P=0.040),medication time(χ~2=9.810,P=0.020)and medication reminder(χ~2=18.161,P<0.001)had different medication adherence.There was a statistically significant difference in the compliance of medication adherence between different hypertension knowledge(χ~2=46.160,P<0.001).6.Multivariate logistic regression analysis found that knowledge of hypertension and medication reminders were the influencing factors of drug compliance in hospitalized patients with hypertension.Patients with knowledge of hypertension have a good adherence rate of 6.746 times to those with poor knowledge of hypertension(95%CI(1.989~22.876));patients with medication reminder have 2.039 times the possibility of medication compliance to those with no medication reminder(95%CI(1.116~3.726))Conclusion:1.This study found that among the 6 types of antihypertensive drugs,β-RB,CA and compound preparations were used at a higher ratio.Most people in the study took a combined medication regimen to control blood pressure,among them,the two drugs regimen are the main ones,followed by the triple drugs,and at least the four drugs.Due to the reasonable ratio of compound preparations,convenient use and good antihypertensive effect,it is recommended to promote.2.The proportion of patients with good medication compliance was 16.6%,lower than relevant domestic and foreign studies,suggesting that appropriate measures should be taken clinically to improve medication compliance of patients with hypertension.3.The knowledge level of hypertension and medication reminder are the influencing factors of medication compliance of hospitalized hypertension patients. |
