| Background:Elderly chronic cor pulmonale right heart failure(chronic oobstructive pulmonary disease,or COPD is its most important primary disease)is a common and serious diseases for respiratory and circulatory system in our country,and there has been a rising incidence of a disease,a serious threat to public health.The disease often merge with high uric acid hematic disease and positive correlation with disease severity.Pulmonary hypertension and myocardial damage is chronic cor pulmonale occurrence and development of the most important and the most basic pathological basis.Endothelin(ET),is a very important factor to be involved in the occurrence and development of pulmonary hypertension.Timely and effective treatment for patients is very important,can reduce symptoms,improve quality of life,reduce mortality.Present situation of treatment:the lack of unified fixed therapeutic schedule;a single treatment often can not achieve the desired effect,adopt combination therapy more programs;New targeted drug treatment of pulmonary hypertension is still need further study in order to better serve the clinical;lung transplantation or heart-lung transplantation is not widely carried out;gene therapy is a lack of broad literature reported that need to be further exploration and research;combine traditional chinese and western medicine treatment is a kind of special treatment way in our country.Conventional treatment the overall effect is still poor,seriously affect the prognosis of patients.So exploration has both can improve the efficacy and security is guaranteed of drugs or measures is of great significance.In recent years,there were some studies have shown that ferulic acid piperazine piece is a new type of endothelin receptor antagonist.Currently,there are occasional reports on the treatment of piperazine feruate for acute exacerbation of pulmonary heart disease,but no reports on the treatment of right heart failure combined with hyperua in elderly patients with chronic pulmonary heart disease have been reported,Therefore,considering try out the ferulic acid piperazine piece treat senile chronic cor pulmonale right heart failure patients caused by COPD and merge with high uric acid hematic disease,to observe its practical curative effect and safety.Objective:To explore the effects of ferulic acid piperazine piece therapy on right heart functions?blood gas analysis and blood uric acid data in elderly patients with chronic cor pulmonale right heart failure caused by COPD and combined high uric acid hematic disease,and provide a reference for the safety of the drug.Methods:The random coin toss method was used to randomly devide 96 elderly patients with chronic cor pulmonale right heart failure caused by COPD and combined high uric acid hematic disease into control group(conventional western medicine,48 patients)and treatment group(conventional western medicine and ferulic acid piperazine pieces,48 patients),and patients were treated for 8 weeks totally.Right heart function parameters(TAPSE?E/A?RVMPI?PASP)?blood gas analysis results(PaO2,PaCO2)?blood uric acid and blood triglycerides were determined for both groups before treatment.After began to treatment for each patientgeneral:Every 2 weeks to review TAPSE,E/A,RVMPI,PASP,weekly review PaO2,PaCO2,review UA blood,blood TG every 4 weeks(But during the treatment if the signs and symptoms obviously abnormal changes in the patients with at any time to review the above indicators).Record the clinical curative effect of two groups of patients with respective,and mainly at the end of 8 weeks of treatment time on curative effect comparison between two groups of patients,at the same time observe adverse drug reactions.Results:After 8 weeks after treatment in both groups right heart function index:TAPSE and RVMPI were improved,the degree of the treatment group improved more obviously thaN the control group(P<0.05),E/A and PASP level difference is not significant,and two groups of patients before and after treatment and after treatment between two groups of level difference is also not significant(P>0.05);.the artery blood gas analysis indicators(PaO2?PaCO2)have improved significantly than before treatment,and the index of the treatment group patients get better degree and more obvious than the control group(P<0.05).Also found that ferulic acid piperazine tablets have fall blood uric acid and blood triglycerides action.Total effective rate was 93.75%in treatment group,significantly higher than 79.17%of the control group(P<0.05).No significant adverse drug reaction was observed.Conclusion:Ferulic acid piperazine piece used in the treatment of senile chronic cor pulmonale right heart failure caused by COPD in patients with high uric acid hematic disease to a certain extent,improve the clinical curative effect,but security is also high,so can be recommend applied clinically.But its overall treatment effect is still limited,still need to continue to actively study and explore. |
