Development Of The Patient Safety Competency Nurse Evaluation Scale

Background:Patient safety is a severe public health issue worldwide and is a core part of the quality of medical care and one of the six goals of nursing quality.Nurses provide health care for patients directly,and their patient safety competency affects patients'safety and overall rehabilitation directly,which is one of the prerequisites for ensuring high quality care.However,there is no a unified and comprehensive understanding of patient safety competency,which is not conducive to further research and application of clinical practice.So it is particularly important to clear the connotation of the patient safety competency under the background of Chinese culture,identify and evaluate nurses'patient safety competency scientifically,discover the advantages and disadvantages in the work and improve the quality of nursing and ensure patient safety.Objectives:To clarity the connotation of the patient safety competency,develop the patient safety competency nurse evaluation scale for nurses in China and test the reliability and validity of the scale.Methods:On the basis of Canadian safety competency framework,the literature review,semi-structured interviews and expert meeting were used to clarify the connotation and component of the patient safety competency of nurses and formed first draft of the scale.The items were selected by using five methods among 302 clinical nurses in five tertiary general hospitals in Beijing.The scale was psychometrically tested among 400 clinical nurses in five tertiary general hospitals in Beijing.Results:1.?The patient safety competency is the ability of health care professionals that reducting of risk of unnecessary harm associated with healthcare to an acceptable minimum,including patient-centered care,safety risk management,evidence-based nursing practice,clinical practice,patient safety culture and continuous quality improvement in six aspects.?The item pool included 104 items.Through 10 experts and 30 clinical nurses selection,the first draft of the scale was formed with 44 items,6 aspects.2.(DAfter item selection,the formal scale contained 35 items.?The content validity of the formal scale was 1,six common factors were extracted by exploratory factor analysis,named respectively:clinical practice,patient-centered care,safety risk management,evidence-based nursing practice,patient safety culture,and continuous quality improvement,the cumulative variance contribution rate was 65.427%.Confirmatory factor analysis confirmed that the 6-factor model fitted well.(X2/df=2.742,RMSEA=0.064,RMR=0.025,GFI=0.816,NFI=0.868,IFI=0.912,CFI=0.911,TLI=0.900).The results showed that the score of the new scale was medium related to the score of Chinese registered nurses' ability scale,r=0.63(P<0.001).?The Cronbach's alpha coefficient of the total scale was 0.902,and Cronbach's of each dimension were between 0.764 to 0.912.The half-confidence of the total scale was 0.823,The retest reliability of the total table was 0.832.?The efficiency and response rate of the scale were higher than 85%,and the average response time was 8.6 minutes.Conclusions:In this study,the connotation of the patient safety competency of nurses was clarified,the prepared patient safety competency nursing evaluation scale was scientific and feasible,and had good reliability and validity.The study provided suggestions for comprehensive evaluation of nurse patient safety competency level in the future.However,it is necessary to set up the norm and further development to improve the scale through multi-center stratified sampling.
