The Study Of Revising And Verifying The Nursing Practice Information Competency Assessment Scale

Objective The aims of this study are introducing the Canadian Nursing Informatics Competency Assessment Scale(C-NICAS),translating it into Chinese version,and testing the psychometric properties.The Chinese version called Nursing Practice Information Competency Assessment Scale,was using to evaluate the current situation of clinical nurses’ informatics competencies in tertiary hospitals and the influencing factors will be analyzed.At the same time,the triadic reciprocal deterministic model of clinical nursing information competency will be constructed based on Bandura’s triadic reciprocal determinism.MethodsPart 1: After acquiring authorization,the C-NICAS was translated into Chinese according with Brislin’s classical translation model.Expert consultation and pre-experiment were used to carry out cross-cultural adjustment of the Nursing Practice Information Competency Assessment Scale,the content validity was also estimated by the consulting experts.Part 2: A survey investigated 953 clinical nurses in three branches of a tertiary hospital in Qingdao by using the electronic Nursing Practice Information Competency Assessment Scale.The software,Excel,SPSS,AMOS,were used to analyze the psychological characteristics of the scale,which contains the item analysis(extreme value method,correlation coefficient method),structural validity(exploratory factor analysis,confirmatory factor analysis),reliability(internal consistency,split-half reliability,retest reliability).Part 3: Based on the triadic reciprocal determinism,the electronic version of Nursing Practice Information Competency Assessment Scale,Computer Self-Efficacy Scale and Clinical Nursing Information System Effectiveness Evaluation Scale were surveyed in 518 clinical nurses from 46 tertiary hospitals in some provinces and municipalities in China.SPSS was used to analyze the general information,the situation of the nursing information competency of those clinical nurses,the significant influencing factors of the clinical nursing information competency and the multiple linear regression equation also be figured out.AMOS was used to construct the triadic reciprocal deterministic model of clinical nursing information competency.ResultsPart 1: After 16 language adjustments were completed,the Nursing Practice Information Competency Assessment Scale consisting of 21 items and 4 dimensions(basic information and communication technology competency,information management competency,ethics and regulations,and nursing practice competency)was initially formed.It’s I-CVIs were0.86~1.00,S-CVI / AU = 0.714,S-CVI / Ave = 0.960.Part 2: The critical ratios of 21 items are ranged from 8.794 to 23.496(P< 0.05),and the correlation coefficient between the scores of each item and the total score of the scale are ranged from 0.535 to 0.801(P< 0.05).Four communalities were extracted by exploratory factor analysis,and the variance contribution rate was 70.445%.The structural equation model built by confirmatory factor analysis was modified(MI = 4),the parameters wereχ~2/df = 2.862,GFI = 0.893,CFI = 0.947,IFI = 0.948,NFI = 0.92,RMSEA = 0.067.The Cronbach’s α coefficient of the Nursing Practice Information Competency Assessment Scale was 0.950,for the dimensions ranged in 0.783~0.925;the Cronbach’s α coefficient of split-half reliability was 0.868,for the dimensions ranged in 0.783~0.889,and the retest reliability was r =0.741,for the dimensions ranged in 0.531~0.862.Part 3: The total score of nursing information competency of clinical nurses was 63.53±8.78,and the average score of items in each dimension from the highest to the lowest was ethics and regulations 3.16±0.18,basic information and communication technology competency is3.06±0.03,nursing practice competency 3.01±0.09,information management competency2.90±0.14.Diploma,computer self-efficacy,effectiveness of nursing information system and computer rank had significant influence on clinical nursing information competency,which explained the variation rate of 34.8% of clinical nursing information competency.The regression equation was Clinical nursing information ability = 25.630 + 0.359 × clinical nursing information system effectiveness + 0.301 × diploma + 0.201 × computer self-efficacy + 0.114 × computer rank.The triadic reciprocal deterministic model of clinical nursing information competency was modified(MI = 4),and the parameters were χ~2/df =2.383,GFI = 0.969,CFI = 0.984,IFI = 0.984,NFI = 0.973,RMSEA = 0.052;R2 of three latent variable in the model were less than 0.50,and the significant influence paths discovered in this study were as follows: "nursing information system → personal factors","personal factors → nursing information competency",and "nursing information system →nursing information competency".Conclusions(1)The Nursing Practice Information Competency Assessment Scale has concise items and easier to understand,it suitable for all levels of clinical nurse;(2)it has great psychometric properties;(3)the nursing information competency of clinical nurses in China is better than the middle level,but the competencies in information management is deficient;factors of diploma,computer self-efficacy,effectiveness of nursing information system and computer rank have influence on clinical nursing information competency;the triadic reciprocal deterministic model of clinical nursing information competency highly corresponds to the triadic reciprocal determinism in Bandura’s social learning theory,which indicates that the Nursing Practice Information Competency Assessment Scale has theoretical usability.
