| Objective:Carotid artery stenosis is an important risk factor for ischemic stroke.Carotid artery stenting has become one of the main treatment methods for carotid stenosis as the development of neural intervention technology.In-stent restenosis is the most common complication of carotid stenting affecting the long-term outcome and influences the long-term efficacy of this technique. The aim of this study is to investigate the value of PALMAZ balloon-expandable stent in in-stent restenosis after carotid artery stenting. Methods:A total of 340 patients underwent placement of carotid artery stent from April 2013 to January 2017 in our hospital.Significant in-stent restenosis(>50%)was found in 16 patients during follow-up visits and 8 of them underwent a PALMAZ balloon-expandable stent implantation.Patients underwent digital subtraction angiography after the secondary stent implantation. Results:Eight patients underwent digital subtraction angiography after the secondary stent implantation. The vascular restenosis rate of eight patients were all less than 10%after the secondary stent implantation. There were better improvement of clinical symptoms after the secondary stent implantation. However, eight patients had unstable hemodynamics, defined as bradycardia or hypotension during the intraoperative period. The symptoms disappeared after proper treatment of atropine, dopamine and other drugs. All the eight patients were followed up by ultrasonography at 3 months and 6 months after the secondary stent implantation indicating that the blood flow of the carotid arteries was smooth and no restenosis was found. In addition, all the patients did not have obvious adverse events such as ischemic stroke, transient ischemic attack, reperfusion injury, myocardial infarction and death. Conclusions:PALMAZ balloon-expandable stent can be a safe, effective and operable choice for the treatment of in-stent restenosis after carotid artery stenting.Further study of the effect of balloon-expandable stent is warranted. |
