Objective:With the constant progress of science and technology,laser has been widely used in the applications of oral disease.At present,studies of chronic periodontitis basis for laser have a rising trend,however,there are different views in laser treatment.In this study we use vivo experiment to observe the short-term curative effect of Er:YAG laser therapies for chronic periodontitis and investigate whether Er:YAG laser irradiation can relieve pain in patients with chronic periodontitis.While,to observe the roots roughness of the vitro teeth,which have been treated by Er:YAG laser.Methods:In vivo parts,we found 40 patients in Shandong Province,Qianfo Hill hospital,who had been selected according to the standard.The patients were randomly divided into observation group(20cases)and control group(20cases);the hospital ethics committee had approved this study,while patients had signed the informed consents.The patients were using piezoelectric ceramic type scaler,EMS A tips for subgingival ultrasonic scaling,after 1 weeks,the control group only use EMS PS for the tip of subgingival Ultrasonic Subgingival Scaling and root planing,while the Ultrasonic Subgingival Scaling of observation group were irradiated by Er:YAG laser in the bag after root planing all treatment,operations were completed by the same doctor.After 1,6,12 weeks treatment,another physician,who had unawared of the treatment,had inspected the whole dentition,indexes including plaque index(PI),bleeding index(BOP),probe the clinical periodontal attachment loss(CAL).We used the visual analogue scale(VAS)on the degree of pain scoring record to detect the degree of painess of all patients at different times(in the course of treatment,after treatment 24h,after treatment for 1 weeks)and used Q-Q test to expressed the normality test measurement data;measurement data of normal distributed showed by x + s.We use variance analysis to detect the difference scaling indexes(before and after treatment)between single and more groups,which have repeated measurement data.Data of the same group(before and after treatment)was compared with Student t-Test or variance analysis;the pain difference(in the treatment,24h after treatment,1 week after treatment)between the two groups analyzed as Student t-Test.All statistical procedures were performed in SPSS22.0 statistical software,(P<0.05).In vitro part,12 extracted teeth were divided into three groups:no treatment group(group C),ultrasonic curettage group(S group),ultrasonic scaling + Er:YAG(S+E),sample preparation.Three groups of samples were frozen and dried in the same condition.We used the electron microscope to scan the roots surface of the three groups and used Kruskal-Wallis rank and inspection to compare roughness data with three groups.SPSS 16.0 software was used for data analysis(P<0.05).Results:In vivo experiment:1.6 weeks and 12 weeks after treatment,PI,BOP and CAL comparative differences between the two groups had no statistical significance(P>0.05)2.Sixth weeks and twelfth weeks after treatment,PI,BOP and CAL were lower than group first weeks after scaling reduced(P<0.05)3.There was no significant difference showed by VAS score between the two groups during treatment,at 24h post-treatment and 1 week post-treatment.(P>0.05)In vitro teeth part:The roots roughness of three groups had no obvious difference scanned by electron microscopy(SEM).Meanwhile,group S had no obvious scratch.Conclusions:In vivo experiment:1.The thorough cleaning,scaling and root planing can control inflammation of Chronic periodontitis.Patients should cooperated with doctor at the stage of supportive periodontal therapy.2.Using Er:YAG laser,which assisted with ultrasonic scaling,scaling,root planing,had no obvious improvement in short-term curative effect towards the treatment of chronic periodontitis patients.3.Whether in the treatment or after the treatment,Er:YAG laser assisted ultrasonic cleaning and scraping treatment of chronic periodontitis can not reduce the degree of pain.In vitro teeth part:Er:YAG laser assisted ultrasound treatment can not change the roughness of the root surface.
