Background: Discoid Lateral Meniscus(DLM)is a kind of meniscus heterogeneity which is wider and thicker than normal meniscus.The mechanism of DLM is still unclear.The current recognition is the congenital developmental stagnation hypothesis.Wantanabe classification is the most frequently used type of Discoid Lateral Meniscus(DLM).Whether the meniscus margin reaches the intercondylar fossa or not and the shape of the meniscus attachment to the posterior border of the humerus is normal or not,can divided DLM into three types: 1.Comp lete type: the free edge of the meniscus reaches the intercondylar fossa;2.Incomplete type: th e free edge of the meniscus does not reach the intercondylar fossa;3.Wrisberg-ligament type:Lack of posterior meniscus-tibia ligament,not attached to the posterior border of the humeru s.The intact discoid meniscus generally does not have clinical symptoms.If there is a tear or injury occurred in the meniscus,there will be clinical symptoms such as knee pain and disc omfort,clicking of joints,claudication,and joint lockness.The use of knee X-ray to diagnose disc DLM is very difficult and can only be confirmed by indirect manifestations such as the distance between the lateral femoral condyle and the tibial plateau and the height of peroneal head.while MRI(Magnetic Resonance Imaging)has a high diagnostic rate,as long as in the sagittal plane of MRI three or more consecutive scan sequences of 4-5 mm appear to be conn ected to the anterior and posterior angles of the meniscus,it can be diagnosed of the discoid meniscus.The discoid meniscus which is not damaged and without clinical symptoms such as limping and clicking does not require surgical intervention.However the discoid meniscus is found to be ruptured or injured,the surgical treatment should be performed.More and more d octors realize that discoid meniscectomy can lead to worsening cartilage wear and osteoarthri tis of the knee joint,and meniscectomy has become a new hot spot.At present,arthroscopic menisceoplasty(retaining a stable peripheral margin)has been recommended by many expert s.The aim of arthroscopic meniscus plasty is to create a stable,normal-shaped meniscus,so a s to increase the degree of matching between knee joint surfaces,and reduce the damage of k nee joint elastic structures during daily activities.Arthroscopic menisceoplasty is the preferre d treatment for all stable discoid meniscus patients with conditional meniscus margin preserv ation.OBJECTIVE: Objective to explore the therapeutic effect and influencing factors of arthrosc opic treatment of discoid meniscus injury.METHODS: Patients with discoid meniscus lesions admitted to the First Affiliated Hospital of Anhui Medical University from April 2015 to September 2016 were selected.There were 115 patients with discoid meniscus: 43 males and 72 females;54 patients on the left side and 61 cases on the right side;age: 5-78 years old;preoperative evaluation using knee joint Lyshol m score Patient knee function.The average preoperative score was 65.8±9.39.The meniscus p lasty / meniscus subtotal resection was performed,and postoperative exercise was guided.Aft er 6 months to 18 months of follow-up,Lysholm score was used to evaluate the postoperative efficacy and was divided into excellent group(score < 85)and unsatisfactory group(score < 85).Result:Patients with rupture of DLM had a Lysholm score of(65.8±9.39)before operation,a nd the follow-up period was 6 to 18 months.The Lysholm score of the knee joint was(89.15±7.45),and the score was before and after surgery.The paired T test showed that there were significant differences in the Lysholm score before and after operation and in the Lysholm sc ore before and after operation.After statistical analysis,the course of disease is related to the curative effect.Age and cartilage damage are closely related to the curative effect.Conclusion:Arthroscopic treatment of discoid meniscus injury is a safe and effective surgical method.The age and degree of cartilage injury are closely related to the outcome.
