Clinical Observation On Treatment Of Early Necrosis Of Femoral Head With Qi Stagnation And Blood Stasis Type With Taohong Siwu Decoction

Objective:through the clinical observation of Taohong Siwu decoction in the treatment of early femoral head necrosis of Qi stagnation and Blood stasis type,compared with Gushi recipe,to provide further guidance for clinical practice.Methods:108 patients(181 hips)with early femoral head necrosis who met the inclusion criteria and were willing to take conservative treatment of traditional Chinese medicine,collected from December 2017 to August 2018 in the Department of Orthopaedics,affiliated Hospital of traditional Chinese Medicine of Xinjiang Medical University.There were 53 cases in the experimental group and 55 cases in the control group.In the experimental group,21 cases(32 hips)were in ARCO IIA stage,22 cases(42 hips)in ARCO IIB stage,10 cases(16 hips)in ARCO IIIA stage.In the control group,23 cases(34 hips)in ARCO IIA stage,23 cases(42 hips)in ARCO IIB stage and 9 cases(15 hips)in ARCO IIIA stage.Including 66 male and 42 famale,age ranged from 30 to 72 years,with an average age of 49.58 ± 9.46 years.The patients were divided into experimental group and control group according to the treatment plan.The experimental group was treated with Taohong Siwu decoction,while the control group was treated with Gushi recipe.VAS pain score,hip Harris score,hip range of motion,SF-36 health scale score and imaging findings were evaluated before treatment,3 months and 6 months after treatment.Results: there were 2 cases lost in the experimental group,2 cases took THA,4 cases lost in the control group and 1 case took THA.Finally,99 cases were obtained,48 cases in the experimental group and 51 cases in the control group.The VAS pain score,HHS and ROM in two groups were better than those before treatment,the difference has statistically significant,P<0.05.There was no statistically difference in SF-36 health scale score before and after treatment in two groups,P>0.05.The HHS and AROM in the experimental group were better than those in the control group,the difference has statistically significant,P<0.05.There was nostatistically significant in VAS pain score and PROM between the two groups,P>0.05.Collapse degree has no statistically significant in two groups before and after treatment,P>0.05,there was no statistically significant between the two groups,P>0.05.The necrotic extent enlarged than before in two groups,P<0.05.There was no significant difference between the two groups,P>0.05.Conclusion:both groups have influence on improving VAS pain score,HHS and ROM.The experimental group was superior to the control group in improving HHS.Both groups could prevent further collapse of femoral head in some way.
