Part 1 Impairment of attention network function during the interictal period of migraine without auraObjective Migraine is a common primary headache in clinical and characterized by recurrent episodes.Patients often complain of memory and attention loss,which seriously affects normal life.At present,a large number of studies have reported the relationship between migraine and attention function,but the results are still inconclusive.Currently,Posner proposed attention network theory and divided it into three domains:alerting,orienting,and executive control network.According to this theory,Fan et al.have designed the attention network test(ANT),which was used to detect the function of three subnetworks and was widely used in many diseases.Therefore,this study was designed to assess whether patients with migraine without aura(MwoA)have impaired attention network function during the interictal period,and analyzed the correlation between headache characteristics(e.g.,history,frequency,duration of each attack and headache score)and each indicator.Methods According to the 2013 International Classification of headache disorder 3?edition(ICHD-3?)included 32 patients with MwoA and 32-age,sex and educational level matched healthy controls(HC).Firstly,the Mini Mental State Examination(MMSE),Zung's anxiety and depression scale(SAS/SDS)was used to exclude the patients with mental and emotional disorders.Then,ANT was used to detect the attention network function of MwoA and HC.Furthermore,two groups were tested by Stroop color word test(SCWT)and Shape Trail test(STT).Finally,Spearman correlation was used to analyze the relationship between headache characteristics(e.g.,history,frequency,duration of each attack and headache score)and each indicator.Results ANT datas showed that the average response time of MwoA(135.47±59.69)ms was significantly longer than that of HC(101.08 ±42.82)ms(P=0.010)in executive network,whereas no significant differences were observed in alerting and orienting network between groups.The average response time of MwoA patients(71.81±13.9;21.49±7.40)ms also exhibited significantly longer than that of HC(60.04±8.95;16.06±4.45)ms in the SCWT C and STT B(P=0.030;P=0.001).Spearman's analysis revealed positive correlations between the average response time of executive control network(r=0.355,P=0.046;r=0.595,P=0.000)and SCWT C(r=0.506,P=0.003;r=0.640,P=0.000)and both frequency and duration of migraine attack,between the average response time of SCWT A(r=0.406,P=0.021),SCWT B(r=0.412,P=0.019)and STT B(r=0.378,P=0.033)and the duration of migraine each attack.Conclusion MwoA patients demonstrate impairment of the attention network during the interictal period,specially in executive control network,which appear to be exacerbated by more frequent and longer migraine attacks.Part 2 The comparative on attention network function of tension headache and migraineObjective Tension-type headache(TTH)and migraine(MG)are common types of primary headache in neurology clinics.Although many studies have confirmed that patients with headache have impaired attention function,there is no research on the attention network function of TTH and MG.Therefore,we intend to use ANT to compare the attention network functions of TTH,MG patients and HC groups.Methods We recruited 41 TTH and MG patients(8 migraine with aura,33 migraine without aura)respectively and 41 age,gender and education matched HC group.All subjects completed SDS,SAS,MMSE,and PSQI prior to ANT.The average reaction time and accuracy of the three sub-networks were then recorded using ANT.Results Our results showed that TTH patients(17.69± 33.01)ms exhibited significantly shorter response time in the alerting network compared to HC(41.17±36.37)ms(P=0.007),whereas no significant difference was observed between MG(30.09±38.09)ms and HC.For orienting network,there were no significant differences between TTH,MG and HC groups(P>0.05).In terms of executive control network,the average response times of MG(125.1±66.55)ms and TTH(118.48±56.48)ms were significantly longer than that in HC group(88.08 ±40.90)ms(P<0.05).There was no significant difference between MG and TTH(P=0.05;P=0.048).For accuracy,there were no significant differences among the three groups(P>0.05).Conclusion Patients with headache had impaired attention network function,tension-type headache is significantly impaired in alerting network and executive control network and migraine is significantly impaired in executive control network.
