| Objective: By observing the clinical curative effect before and after the treatment of diabetic nephropathy(both yin and yang deficiency)and the effect of the function of renal tubular reabsorption and the function of tubular reabsorption,the clinical basis of dkd is enriched by the observation of the effect of the kidney and the spleen on the treatment of diabetic nephropathy(both yin and yang deficiency).Methods: from December 2017 to December 2018,60 patients with diabetic nephropathy with deficiency of yin and yang were randomly divided into treatment group(30 cases)and control group(30 cases),.The control group received the basic treatment of the diabetic nephropathy in western medicine.The treatment group was treated with Jianpi Bushen recipe on the basis of routine western medicine treatment for 12 weeks.The improvement of TCM symptoms and the glomerular filtration index of ?1-MG,?2-MG,RBP and the functional index of the renal tubular reabsorption function is NAGwere observed in the two groups.The clinical effect of Jianpi Bushen Recipe on the treatment of diabetic nephropathy(both yin and yang deficiency)was evaluated by the statistical method.Results:1.There was no significant difference in general conditions(sex,age,course of disease,combined medication,blood routine,liver function,kidney function,FBG,2h PBG,blood biochemistry,urine biochemistry)between the two groups before treatment(p > 0.05).It has clinical comparability.2.evaluation of therapeutic efficiency:(1)Total score of TCM symptoms: The total effective rate was 93.3% in the treatment group and 73.3% in the control group.There was significant difference between the treatment group and the control group(p< 0.05).(2)Score of single symptom of traditional Chinese medicine: After treatment,the scores of eight symptoms in the treatment group were significantly lower than those in the control group(P <0.05),and the scores of eight symptoms in the treatment group were significantly lower than those in the control group(P<0.05),and the scores of eight symptoms in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,the scores of eight symptoms in the treatment group were significantly lower than those before treatment(P< 0.05).After treatment,the scores of eight symptoms of cold fatigue,dry mouth and throat,less food intake,long urination,acid tinnitus,loose stools,dry knot of stool and five upset heat were significantly lower than those before treatment(P<0.05).In the control group,the scores of cold fatigue,dry mouth and pharynx,less food,acid tinnitus and long urination were significantly lower than those before treatment(P<0.05),while dry stool and loose stools were found in the control group(P<0.05),and the scores of cold fatigue,dry mouth and dry throat,less food in the control group were significantly lower than those before treatment(P<0.05).There was no significant improvement in the score of symptoms of five heart irritation and heat compared with that before treatment,and there was no significant difference between the two groups(P>0.05).After treatment,the scores of seven symptoms in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,the scores of cold fatigue,dry mouth and pharynx,less food consumption,loose stool,five upset heat,lumbar acid tinnitus and stool dry knot were significantly lower in the treatment group than in the control group(P <0.05).3.Total curative effect: the total effective rate was 90% in the treatment group and 73.3% in the control group,and there was significant difference between the treatment group and the control group(P<0.05).4.Laboratory index: after treatment,FBG,in the treatment group was significantly higher than that in the control group(P<0.05).There was no significant difference in blood glucose and Hb Al C,SCr,GFRbetween the control group and the control group at2 h PBG(P>0.05).The blood ?1-MG,blood ?2-MG,urine NAG,BUN and 24 h urine protein quantity in the treatment group were significantly lower than those in the control group(P<0.05).After treatment,there was no significant change in blood RBP between the treatment group and the control group(P>0.05),and there was no significant difference between the treatment group and the control group.Safety index evaluation: there were no adverse reactions in both groups before and after treatment.Conclusion:1.Bushen Jianpi recipe combined with western medicine can improve the clinical symptoms of DKD patients with deficiency of yin and yang,and its effect is better than that of using western medicine alone.It can improve the treatment experience of patients and improve the quality of life of the patients.2.Bushen Jianpi recipe combined with western medicine can reduce blood urea nitrogen index and 24 h urine protein quantity in DKD patients,and its effect is better than that of using western medicine alone,suggesting that Bushen Jianpi recipe can improve part of renal function;3.Bushen Jianpi recipe can improve part of renal function;Bushen Jianpi recipe combined with western medicine could reduce the levels of ?1-MG,and ?2-MG,urine NAG in diabetic nephropathy patients,and improve the glomerular filtration index and renal tubule reabsorption index in diabetic nephropathy patients.
