BackgroundThe most of diffuse axonal injury(DAI)survivors always left behind the cognitive impairment to some degree,which mainly manifested in “high level” cognitive impairment such as memory,attention and executive function.At present,how to achieve early cognitive assessment of DAI patients is the problem faced by most of neurosurgeon.In the individual,some brain regions,which have structural and functional correlation,constituting a neural network that regulate the organization of the body.Among them,the processing of the “high level” cognitive information often requires the integration of neural networks.Therefore,it is impossible for us to simply explore isolated neurons to answer a lot of cognitive problems.Objective1.We analyzed the change of neural network structure caused by DAI and discussed the application value of neural network structure change in early diagnosis of cognitive dysfunction after DAI based on the default network(DMN).2.To explore the effect of the small world properties of the neural network and recognize the changes in the function of the neural network in the early stage of DAI,and provides a theoretical basis for early diagnosis of cognitive dysfunction after of DAIMethods13 cases of DAI patients and 8 healthy volunteers were included as clinical research subjects during the studying period.Diffusion tensor imaging(DTI)technology was used to evaluate the structure of neural network.DTI sequence images were imported into GE-AW4.4 workstation,and the Functool software was used for post-processing.The function of the neural network is evaluated by the resting-state functional magnetic resonance(fMRI)technology,and the fMRI data is introduced into the GRETNA software for post processing.The Victoria Stroop test(VST)were used for the assessment of attention,Hopkins verbal learning Test-Revised(HVLT-R)and the digit span test(DST)were used for the experimental object memory evaluation,the connection test trail making test(TMT)were used for the executive function of subjects.SPSS22.0 software was used for data processing and analysis.Results1.There was a significant difference of the imaging parameters between DMN and DAI in patients during the acute stage,which mainly related to the corpus callosum,cingulate gyrus,frontal and occipital fascia.2.The scores of the attention scale,the working memory scale and the executive function scale were statistically significant compared with the DAI group and the control group.3.The change of DMN structure in acute phase of DAI group is related to the attention scale and the working memory scale in the recovery state,but it shows no correlation with the score of executive function scale.4.Although the neural network of patients still shows the small world property after DAI,the small world property change has statistical difference compared with the control group.5.The attribute values of neural network small world changed after DAI,in which the average shortest path change had statistical difference compared with the control group.Conclusions1.DAI can cause damage to the structure of the neural network,and it can help early diagnosis of cognitive dysfunction after DAI.The destruction of DMN network structure is the basis for early diagnosis of attention and working memory dysfunction after DAI.2.DAI can cause changes in the function of neural network that involved in the regulation of cognitive function,which mainly shows the change in the attributes of the small world.The changes of the small world property of neural network may be an important factor in cognitive dysfunction after DAI.
