Objective: This article mainly discusses the application of quantitative indicators of magnetic resonance perfusion imaging in the diagnosis and differential diagnosis of alveolar echinococcosis and brain metastasis,and further improves the diagnostic accuracy of the two kinds of diseases,so as to provide more accurate indicators for clinical treatment.Methods: 10 cases of alveolar echinococcosis and 10 cases of brain metastasis were collected(20 cases,15 males and 5 females,each with 40 effective lesions).Routine brain MR plain scan,enhanced scan and magnetic resonance perfusion imaging were performed respectively.The regional cerebral blood volume(rCBV),regional cerebral blood flow(rCBF),mean transit time(MTT)and peak time(TTP)were measured in the parenchymal area,marginal area,edema area and the contralateral normal brain parenchymal area,as well as in the metastatic brain area,edema area and contralateral normal brain parenchymal area.Results: the cerebral vesicle lesions echinococcosis in parenchyma of rCBV,rCBF,MTT and TTP respectively were:123.29(80.50,171.43)?22.00(18.46,26.52)?10.55(7.49,14.71)?37.94(29.35,45.99).The values of rCBV,rCBF,MTT and TTP in the edge area respectively were:125.33(80.64,205.94)?18.88(12.25,35.50)?10.01(7.44,12.59)?34.50(27.48,39.20).The edema zone of rCBV,rCBF,MTT and TTP value respectively were:125.33(80.64,205.94)?18.88(12.25,35.50)?10.01(7.44,12.59)?34.50(27.48,39.20).The values of rCBF,rCBV,MTT and TTP in parenchymal areas of brain metastases were 401.18(299.32,722.10),100.50(53.09,139.01),11.83(10.10,12.62),26.51(23.80,32.91),and 208.82(106.58,248.84),23.14(17.37,41.30),9.90(7.91,12.33),27.93(29.29),respectively.There were significant differences in rCBV,rCBF and TTP between the parenchymal areas of alveolarechinococcosis and the marginal areas,edema areas and contralateral non-abnormal areas.There were significant differences in parenchymal area,edema area and control area of brain metastases.There were significant differences in rCBF,rCBV and TTP between the parenchymal areas of brain hydatid lesions and the parenchymal areas of brain metastases.There were significant differences in rCBF and rCBV between the marginal zone of brain hydatid lesions and the parenchymal areas of brain metastases.Conclusion:MR perfusion imaging can provide valuable diagnostic basis for the diagnosis and differentiation of alveolar echinococcosis and brain metastases.
