Objective Pregnancy with gynecological malignant tumors is a rare complication of pregnancy,of which pregnancy with cervical cancer and pregnancy with ovarian cancer are the most common;pregnancy with tubal cancer,pregnancy with vulvar cancer and so on are extremely rare.It has obvious clinical characteristics of difficult diagnosis and treatment.There are still no clear clinical guidelines to regulate the diagnosis and treatment of this disease.From October 2005 to December 2018,22 cases of pregnancy complicated with gynecological malignant tumors in five hospitals including Dalian Maternal and Child Health Hospital were collected.Through systematic analysis of clinical data,to explore the advantages and disadvantages of various diagnostic methods for pregnancy complicated with gynecological malignant tumors and summarize the best diagnostic methods;To analyze the cases of misdiagnosis and missed diagnosis and reduce the misdiagnosis rate of clinical diagnosis and treatment;To analyze the clinical characteristics and outcomes of pregnancy complicated with gynecological malignant tumors,and to provide the basis for the best treatment.Methods Retrospective analysis of clinical data of 22 pregnant women with gynecologic malignancies admitted to five hospitals in Dalian from October 2005 to December 2018,and pay a return visit to patients and their offspring.Among them,11 cases of pregnancy with cervical cancer,11 cases of ovarian cancer.The clinical data of pregnancy with cervical cancer and ovarian cancer were analyzed,and the data were analyzed by SPSS21.0 statistical software.Results The incidence of pregnancy with gynecologic malignancies is 0.09‰(22/241770),among them.11 cases of pregnancy with cervical cancer,11 cases of ovarian cancer.Among the pregnancy with cervical cancer,8 cases were given standardized treatment after giving up pregnancy,2 patients chose to continue pregnancy,1 case was found after childbirth;There were 4 newborns,including 1 premature infants,up to now,all newborns are healthy.Among the pregnancy with ovarian cancer,7 cases of malignant ovarian tumors.except 1 case of stage Ⅲ C.the remaining 6 cases were stage Ⅰ patients,stage Ⅲ C patients gave up pregnancy,while the other 6 patients chose to terminate pregnancy on the premise of ensuring the survival of the fetus as far as possible;There were 9 newborns,including 4 premature infants,among them,1 cases suffered neonatal asphyxia and died 5 minutes after birth,and the remaining 8 neonates were healthy.Conclusion Pregnancy with gynecological malignant tumors is more common in cervical cancer and ovarian cancer.In the case of pregnancy with cervical cancer,the incidence of cervical cancer in pregnancy is lower than that reported in previous literatures,which may be related to standardized screening:Screening for high-risk groups should be strengthened as the number of older pregnant women increases;Pregnancy with cervical cancer is used to be missed because of the special period;The treatment plan for the disease should be combined with the stage of the disease,the gestational age,and the patient’s wishes.In the case of pregnancy with ovarian cancer,it lacks typical clinical symptoms,the combination of multiple auxiliary examinations is of great significance in the diagnosis of diseases;Diagnosis and staging can be determined according to the intraoperative and postoperative pathological results.most of the tumors are early low-grade malignant;For patients with clinical symptoms of other systems,we should be alert to metastatic tumors;The treatment plan for the disease should be individualized.
