| Objective: With the continuous progress of medical technology,the prevention,early detection and treatment of cervical cancer become possible,but there are a large number of patients with advanced cervical cancer who can not be operated on,and the treatment effect of advanced cervical cancer is still not ideal.Previous clinical trials have shown that the combination of antiangiogenic drugs and chemoradiotherapy is superior to conventional therapy.Endostatin is a new recombinant human vascular endostatin developed in China.Now endostatin combined with chemotherapy has also been used as a first-line treatment for a variety of solid malignant tumors.But,um degree combined chemoradiation can benefit the patients with cervical cancer,the safety of the joint application of research literatures have not been reported,so this topic plans to begin from clinical specimens,analysis of VEGF and b FGF expression before and after combined radiation and chemotherapy in the grace of cervical cancer and with nearly period curative effect,clear grace degrees whether can further enhance the sensitivity of the radiation to treat cerical cancer,aiming at providing a new drug in the treatment of cervical cancer and provide scientific basis for clinical.Methods: 60 patients with stage IIB-IVA cervical cancer admitted to our hospital from July 2017 to November 2018 were selected,including 30 patients in the experimental group and 30 patients in the control group.The experimental group was treated with endor synchronous radiotherapy and chemotherapy,while the control group was treated with radiotherapy and chemotherapy alone.Pelvic external intensitymodulated radiotherapy was treated with linear accelerator 6mv-x ray,all patients were positioned under CT line simulation positioning machine,and radiotherapy was treated with rotary intensity-modulated irradiation.Radiation dose: 1.8gy each time,5 times a week,45 ~ 50.4gy /25-28 f.Afterloading treatment was conducted with the threedimensional afterloading treatment machine of hetong company.Afterloading treatment was added in the fifth week,with the dose of HRCTV 5.5 ~6 Gy once a week,4~6 times in total.No external irradiation was conducted on the day of afterloading treatment.Concurrent chemotherapy was given with cisplatin 30 mg/m2,intravenous infusion once a week for 4-5 weeks.The control group received the above standard treatment.In the experimental group,on the basis of standard therapy,continuous infusion of micropump was used 3-5 days before radiotherapy,210 mg/168 h for each cycle of entricity,and 1 time of entricity treatment was given after 2 weeks of rest,a total of 2 times.During the using degree of fasting venous blood were taken espectively before and after chemoradiation therapy 4 ml,1 500 r/min,the centrifugal 10 min,separation of serum,in-70 ℃ refrigerator,30 min before the experiment to take out the balance to room temperature.The serum VEGF and b FGF levels of the enrolled patients before and after treatment were determined by ELISA.Combined with clinical data,the expression levels of VEGF2 and b FGF before and after and the shortterm efficacy were analyzed.Results: 3 months after the end of treatment,11 patients in the control group had complete response(CR),15 patients had partial response(PR),2 patients had stable(SD),and 2 patients had progressive(PD).Clinical effective rate(ORR)87%;In the experimental group,there were 13 cases of complete response(CR),15 cases of partial response(PR),1 case of stability(SD)and 1 case of progression(PD).It can be seen that the clinical efficiency of the experimental group is higher than that of the control group.The levels of VEGF and b FGF in the experimental group and the control group before treatment were all greater than 0.05,with no statistical difference.The levels of VEGF and b FGF in the two groups after treatment were significantly lower than those before treatment,P <0.001.The levels of VEGF and b FGF in the experimental group and the control group after treatment were all greater than 0.05,with no statistical difference.Conclusion: Compared with radiotherapy alone,endo combined with radiotherapy can further down-regulate serum VEGF and b FGF levels in patients with advanced cervical cancer. |
