Objective: To explore the correlation between serum uric acid level and TOAST subtypes and neurological defects in middle-aged patients with acute ischemic stroke,and to provide a theoretical basis for the prevention of middle-aged patients with acute ischemic stroke.Methods:Collected in January 2017-December 2017 in hebei engineering university affiliated hospital neurology of 128 cases of patients with acute ischemic stroke in middle-aged group,the research object of general information collected,including age,sex,hypertension,diabetes history,smoking history,history of drinking,blood lipid(triglyceride,total cholesterol,low density lipoprotein),blood glucose,creatinine,uric acid and carotid intima-media thickness,TOAST classification,degree of nerve function defect.Meanwhile,125 cases of age-matched healthy physical examinees from the same area in the physical examination center of our hospital were collected as the control group,whose basic clinical data and relevant biochemical indicators were collected.According to the improved TOAST classification standard in2007,the case group was divided into five groups: large atherosclerosis group,small artery occlusion group,cardiogenic embolism group,other definite etiology group and unknown etiology group.The degree of neurological impairment was divided into three groups: mild group,moderate group and severe group by the national institutes of health stroke scale.Results:1.Case group and the control group of age,gender,history of heart disease,history of drinking,creatinine,triglyceride and total cholesterol differences by comparing had no statistical significance(p>0.05),in the case group common risk factors such as history of history of high blood pressure,diabetes,smoking history,low density lipoprotein cholesterol(hdl-c),uric acid and strength artery intima-media thickness rate is higher than the control group.The difference was statistically significant(p<0.05).2.The SUA levels of all subjects were divided into four subgroups according to their quarterwise levels for Logistic regression analysis.After adjusting for relevant risk factors,the results showed that the OR values were 1.952,2.107 and 3.590,respectively.The AIS risk of SUA>414.50 subgroup was 3.590 times higher than that ofSUA 266.50 subgroup(p=0.002).3.The influence of SUA level on TOAST typing was analyzed,and multivariate Logistic regression analysis was performed for each typing.After the correction of relevant risk factors,high SUA level was still a risk factor for LAA typing,OR =3.782,p=0.003.For SAO and SUE typing,the analysis showed that SUA level had no statistical significance(p>0.05).4.According to the NIHSS score of patients in the case group and SUA level as the independent variable,Logistic regression analysis was conducted for different degrees of illness.For mild and moderate disease,the analysis results showed that the influence of SUA level on the disease was not statistically significant(p>0.05).Conclusions:Elevated serum uric acid level in middle-aged people may be a risk factor for ischemic stroke;The increase of serum uric acid level in middle-aged people is mainly related to the atherosclerotic stroke of the middle and large arteries in the TOAST type of ischemic stroke.
