Acute Respiratory Distress Syndrome(ARDS)is an acute diffuse inflammatory lung injury.It is clinically proven that high levels of positive end-expiratory pressure ventilation have a certain effect on the treatment of ARDS patients,which can expand the collapsed alveoli..The treatment of ARDS patients mainly adopts BiPAP(Bi-Level Positive Airway Pressure)ventilation mode,which can stably provide inspiratory pressure support in the inspiratory phase and provide expiratory pressure support in the expiratory phase,but Continued high levels of positive end-expiratory pressure ventilation can have many negative effects.In view of the problem of continuous high level of positive end-expiratory pressure ventilation causing secondary damage to the patient’s lungs,this paper innovatively designed a dual-level positive airway pressure(DPAP)model,and Verify its validity.After the search,no one has proposed this ventilation mode,which is an original research.The main research contents of this paper are as follows:(1)In the design of the variable pressure bi-level positive airway pressure positive ventilation mode,the non-invasive ventilator can alternately exhale two different basic pressures and output a certain suction to support the positive pressure on the basic positive pressure.The non-invasive ventilator can alternately output different pressures according to the set action time change.The effectiveness of the new DPAP ventilation model is divided into three parts:simulated lung experiment,normal human experiment and clinical application.The control group was the BiPAP ventilation mode,and the experimental group was the DPAP new ventilation mode.(2)Simulated lung experiment Using the Tess heart and lung biomimetic instrument to simulate normal and ARDS patients,Test Chest Basic software was used to monitor changes in tidal volume and airway pressure under different conditions,and then image processing,data analysis and analysis.Linear regression analysis was performed on the positive pressure and the inspiratory support positive pressure using R x64 3.4.0 software.In the simulated lung experiment,it was verified that the monitoring pressure waveform in the DPAP ventilation mode is basically the same as the design pressure waveform.The basic positive pressure set in this mode provides the pressure support to meet the ventilation demand,and also avoids the patient being in the high pressure state for a long time.(3)In normal experiment,30 samples were used for testing,data was recorded with Flexo SD card,and imported into Flexo ST30 non-invasive ventilator professional software Lotus Auto Manager V331Auto Manager V331 analysis pressure one time,flow rate one time curve judgment invalid trigger,invalid The last expression of the trigger is an invalid trigger rate.Through randomized block analysis of variance and paired t-test,it was verified that different pressure levels,ie different positive inspiratory pressure and positive expiratory pressure,are important factors affecting human-machine synchrony.From the Pearson correlation coefficient,Bland-Altman analyzed the correlation analysis and consistency analysis between the BiPAP ventilation mode of the control group and the invalid trigger rate of the experimental DPAP new ventilation mode,thereby verifying the human-machine synchrony.(4)According to the actual situation of the patient,the DPAP new ventilation mode is applied clinically to observe the changes of arterial blood gas and track the therapeutic effect.It was verified that the application of DPAP new ventilation mode in clinical application was better than that of BiPAP ventilation mode.In summary,the new DPAP ventilation mode is superior to the BiPAP ventilation mode.It is successfully verified that the DPAP ventilation mode can achieve ventilation on two different basic positive pressures according to their respective action times.It is a better ventilator ventilation mode.It is very helpful for the treatment of patients with ARDS.
