Objective:To study the clinical effect of multi-mode jet ventilation and high-frequency jet ventilation in airway management of fiberoptic bronchoscopy under general anesthesia.Methods:40 patients of both sexes,aged 40-82 yr,with body mass index of18.4-29.1kg/m2,of American Society of Anesthesiologists physical statusⅠ-Ⅲ,scheduled for fiberoptic bronchoscopy under general anesthesia,were divided into 2 groups(n=20)by a random number table method:the high-frequency jet ventilation group(H group)and the multi-mode jet ventilation group(S group).Group H adopted high-frequency jet ventilation and group S adopted multi-mode jet ventilation after general anesthesia with laryngeal mask.The HR,MAP,Sp O2of vital signs,the PH value,the partial pressure of arterial oxygen(Pa O2)and arterial carbon dioxide partial pressure(Pa CO2)of arterial blood gas analysis were recorded after patients entering the operating room(T0),15min(T1)and 30min(T2)after high-frequency jet ventilation or multi-mode jet ventilation,restore spontaneous breathing immediately(T3),the concentration of Glu,C-reactive protein(CRP),cortisol and interleukin-6(IL-6)were measured by enzyme-linked immunosorbent assay.The cases whose operation was suspended due to PETCO2>80mm Hg were recorded.Results:Hyoxemia was not found in the two groups.Compared to T0,HR increased at T2,3in both groups,IL-6,Sp O2and Pa O2were increased at T13in both groups,In group H,MAP,blood glucose and cortisol increased at T2,3,the p H value of group H decreased while Pa CO2increased at T1?3,CRP increased at T3in group H,the p H value of group S decreased at T2,MAP,blood glucose and cortisol increased at T3in group S(P<0.05).Compared with group H,HR and MAP were decreased at T2,PH value was increased and Pa CO2were decreased at T1?3,blood glucose and cortisol and IL-6 were decreased at T2,3,CRP was decreased at T3in group S(P<0.05).There are 3 cases in group H and 0 case in group S whose operation was suspended due to PETCO2>80mm Hg.Conclusion:The multi-mode jet ventilation is superior to high-frequency jet ventilation in the airway management of painless fiberoptic bronchoscopy.
