| Objective To investigate the relationship between monocyte/HDL-C ratio(MHR)and hypertension complicated with asymptomatic cerebral infarction(ACI),to explore a new method for ACI risk screening of hypertension patients.Methods A total of 349 patients with hypertension,recruited from the Department of Hypertension,First Affiliated Hospital of Guangxi Medical University from January 2015 to March 2018 were enrolled in this study.They were divided into ACI group(ACI,n=138)and non-complicating ACI group(NACI,n=211)according to the results of CT or MRI.Their data of sex,age,height,weight,blood pressure,course of hypertension,past medical history,medication history,smoking and drinking history were collected.Their results of blood routine,blood lipids,renal function,carotid ultrasonography were also collected to record white blood cell count,neutrophil count,lymphocyte count,monocyte count,total cholesterol,triglyceride,low density lipoproteincholesterol,high density lipoprotein cholesterol and serum creatinine.MHR were calculated based on the recorded data.Logistic regression analysis was used to analysis influencing factors of hypertension complicated with ACI.In addition,receiver operating characteristic(ROC)curve was performed to assess the predictive value of MHR for the occurrence of ACI in hypertension patients.Results 1.There were no significant differences in body mass index,systolic blood pressure,diastolic blood pressure,lymphocyte,triglyceride,low density lipoprotein cholesterol,dyslipidemia and whether to take lipid-lowering drugs between the two groups(P(29)0.05).2.There were significant differences in age,male ratio,course of hypertension,carotid plaque ratio,smoking ratio,alcohol consumption ratio,white blood cells,neutrophils,monocytes,MHR,serum creatinine,total cholesterol and high density lipoprotein cholesterol between the two groups(P(27)0.05).3.Logistic regression analysis showed that age(OR=1.040,95% CI 1.015?1.065,P=0.002),carotid plaque(OR=2.200,95%CI 1.180?4.099,P=0.013),MHR(OR=1.245,95% CI 1.159?1.337,P(27)0.001)and serum creatinine(OR=1.021,95% CI 1.008 ? 1.034,P=0.001)were independent risk factors for hypertension complicated with ACI.4.The cut-off value for MHR predicting the occurrence of ACI was 10.29 with the sensitivity and specificity at 70.3% and 63.0% respectively,the area under curve(AUC)was 0.760(95% CI 0.708~0.812,P< 0.001).Conclusion MHR is significantly correlated to hypertension complicated with ACI,and it is an independent risk factor of hypertension complicated withACI,which affords a valuable indicator for risk screening.Besides,age,carotid plaque and serum creatinine were independent risk factors for hypertension complicated with ACI. |
