The application of virtual touch tissue imagingquantification and contrast-enhanced ultrasound in differential diagnosis of benign and malignant breast massesObjective To explore the value of virtual touch tissue imagingquantification(VTIQ)and contrast-enhanced ultrasound(CEUS)in differential diagnosis ofbenign and malignant breast mass lesions.Methods Sixty-six patients with breastmasses were examined by VTIQ and CEUS to obtain the mean value of shear wavevelocity and contrast-enhanced ultrasound imaging features.To analyze thedifference of mean shear wave velocity and contrast-enhanced ultrasound featuresbetween benign and malignant lesions;To analyze the diagnostic value of VTIQ andCEUS alone and in combination for breast mass lesions.Results 1.Pathologicalresults: 66 cases of breast mass lesions,34 cases of malignancy,32 cases of benign.2.The VTIQ of breast masses measured the SWV value of benign masses(0.98-6.98)m/s and the average SWV value of benign masses(3.14+1.06)m/s;the SWV valueof malignant breast masses was(1.66-9.32)m/s and the average SWV value was(5.44+1.58)m/s.The difference of SWV value between benign and malignanttumors was significant.ROC curve showed that the best diagnostic threshold of VTIQ was 4.07 m/s.The sensitivity,specificity and accuracy of benign andmalignant nodules were 70.6%,87.5%,78.8% with SWV 4.07 m/s as the cut-offpoint.The area under ROC curve was 0.79%.3.Diagnostic results of CEUS in breastmass lesions The CEUS features of benign and malignant breast masses showedsignificant differences in enhancement intensity,enhancement mode,regressionmode,contrast agent distribution uniformity,angiographic enhancement vessels(P< 0.05),and the sensitivity,specificity and accuracy of CEUS in diagnosis of breastmass lesions were 85.3%,53.1% and 69.7%,respectively.4.Diagnostic value ofVTIQ combined with CEUS in breast mass lesions The sensitivity,specificity andaccuracy of combined application of VTIQ and CEUS in the diagnosis of breast masslesions were 91.2%,93.8% and 92.4% respectively.The diagnostic efficacy ofcombined use of VTIQ and CEUS was significantly higher than that of single use ofCEUS or VTIQ(P < 0.05).Conclusion Both VTIQ and CEUS are valuable indifferential diagnosis of benign and malignant breast mass lesions.Thecombination of VTIQ and CEUS can significantly improve the efficiency ofdifferential diagnosis of benign and malignant breast mass lesions and thedetection rate of malignant breast mass.Part ? The application of virtual touch tissue imagingquantification and contrast-enhanced ultrasound in the diagnosis of breast non-mass-like lesionsObjective To explore the value of virtual touch tissue imagingquantification(VTIQ)and contrast-enhanced ultrasound(CEUS)in differential diagnosis of breastnon-mass-like benign and malignant lesions.Methods Eighty-five patients withnon-mass breast lesions suspected to be diagnosed by conventionalultrasonography(US)were selected.The imaging data of US,VTIQ and CEUS werecollected.The value of US,VTIQ,CEUS and VTIQ combined with CEUS indifferentiating benign and malignant non-mass breast lesions was compared andanalyzed according to the pathological results as the gold standard.Results 1.Pathological results 85 lesions,46 benign and 39 malignant.2.Conventional Ultrasound Characteristics Conventional ultrasonographic features of non-massmalignant breast lesions have statistical significance: micro-calcification,bloodflow richness and resistance index;non-mass malignant lesions have a higherproportion of micro-calcification,more abundant internal blood flow and moreresistance index than 0.7.3.VTIQ results The SWV values and SWV ratios innon-mass benign breast lesions were significantly lower than those in malignantbreast lesions [(3.43(+0.59)m/s vs(4.76(+0.95)m/s,(1.73(+0.43)vs(2.67(+0.77)](P < 0.05);the sensitivity,specificity and accuracy of VTIQ in the diagnosisof non-mass breast lesions were 76.9%,80.4% and 78.8%,respectively;4.CEUSresults CEUS of non-mass malignant breast lesions is characterized by highenhancement,fast-in and Fast-out of contrast media,heterogeneous enhancement,and enhanced angiographic vessels around or inside the lesions.The sensitivity,specificity and accuracy of CEUS in the diagnosis of non-mass breast lesions were79.5%,80.4% and 80.0%,respectively.5.VTIQ,CEUS and their combined diagnosticresults The sensitivity,specificity and accuracy of VTIQ combined with CEUS inthe diagnosis of non-mass breast lesions were 89.7%,93.5% and 91.8%,respectively.The combined diagnostic efficacy was significantly higher than that ofCEUS or VTIQ alone(P < 0.05).The diagnostic value of CEUS and VTIQ wassignificantly higher than that of US(P < 0.05).Conclusion CEUS and VTIQ havegood diagnostic value in differential diagnosis of non-mass benign and malignantbreast lesions.VTIQ combined with CEUS has complementary advantages,whichcan obtain more comprehensive diagnostic information of breast lesions andimprove the diagnostic accuracy of non-mass malignant breast lesions.
