Objective To investigate the ultrasonographic features of cervical lymph node metastasis in elderly patients with papillary thyroid carcinoma and related factors in clinical diagnosis.The ultrasound features of two-dimensional ultrasound,color Doppler and elastography were analyzed to compare their diagnostic accuracy for cervical lymph node metastasis in thyroid malignancies.In order to explore the value of ultrasound technology in the diagnosis of malignant thyroid disease and lymph node metastasis from multiple angles.Methods A total of 121 patients with senile thyroid papillary carcinoma admitted to our hospital in the past three years were selected as subjects.The surgical pathological results were used to determine whether cervical lymph node metastasis was divided into metastatic group(n=68)and non-metastasis group(n=53).Comparison of color Doppler ultrasound blood flow parameters between the two groups of metastatic and non-metastatic patients: systolic acceleration time(AT),systolic peak blood flow velocity(PSV),pulsatility index(PI),resistance index(RI)And analyze the correlation between each parameter and lymph node metastasis.At the same time,179 lymph nodes of 121 patients were examined by conventional two-dimensional ultrasound,color Doppler and elastography.The sonographic features of the lymph nodes,color Doppler flow distribution and elastography scores were recorded,and pathology was recorded.Results were compared for analysis.Results In the group of thyroid papillary carcinoma with lymph node metastasis,compared with the non-metastasis group,the systolic acceleration time(PSV)in the metastatic group was significantly accelerated,and the pulsatility index(PI)and resistance index(RI)were significantly increased(P).<0.05),there was no significant difference in systolic acceleration time(AT)between the metastatic group and the non-metastasis group(P>0.05).Correlation analysis showed that PSV,PI,RI in the tumor of elderly patients with papillary thyroid carcinoma There was a significant positive correlation between cervical lymph node metastasis(P<0.05).There was no significant correlation between AT and cervical lymph node metastasis in elderly patients with papillary thyroid carcinoma(P>0.05).The proportion of lymph node and aspect ratio ? 2 in patients with metastatic disease Significantly lower than the non-metastasis group,lymph node calcification,cystic change,aspect ratio <2accounted for significantly higher than the control group(P <0.05),179 lymph nodes in this case,the final pathological results were obtained,the benign diagnosis was 78,The viciousness is 101 pieces.The specificity,sensitivity and accuracy of two-dimensional ultrasound diagnosis of lymph nodes were 76.9%,74.5%,and 75.1%.The specificity,sensitivity and accuracy of Doppler ultrasound in the diagnosis of lymph node metastasis were 82.9%,76.5%,and 81.3%.The specificity,sensitivity and accuracy of elastography in the diagnosis of lymph node metastasis were 92.5%,81.4%,and 87.9%.Conclusion Ultrasound technology has a good application effect in the prediction of cervical lymph node metastasis of malignant thyroid nodules.For elderly patients with papillary thyroid carcinoma,the color flow parameters PSV,PI,RI and cervical lymph node metastasis have significant correlation.The blood flow parameters of the lesions before the operation of the patients can help to determine the lymph node metastasis of the patients..In addition,in the examination of the nature of cervical lymph nodes,elastography shows a high accuracy rate,has a very good clinical guidance,and can be applied for promotion.In summary,both color Doppler ultrasound and elastic ultrasound imaging have shown good application prospects in the diagnosis of cervical metastasis of thyroid cancer.This study aims to lay the foundation for the combined application of ultrasound technology and enhance the clinical application value of ultrasound technology.
