Value Of Color Doppler Ultrasound For Diagnosis Of Metastasis Of Cervical Lymph Nodes In Patients With Thoracic Esophageal Carcinoma

Objective:To study value of color Doppler ultrasound for diagnosis of metastasis of cervical lymph nodes in patients with thoracic esophageal cancer.Method:Retrospective analysis was performed on clinical data of 32 patients who received esophagectomy and three-field lymphadenectomy in department of thoracic surgery of second affiliated hospital of Kunming Medical University between January 2014 to December 2018.Thirty-two patients received color Dopplerultrasonography in the first week before surgery.The pathological results of cervical lymph nodes were taken as the standard to study of color Doppler ultrasonography in the diagnosis of cervical lymph nodes metastasis of thoracic esophageal cancer.Results:Color doppler ultrasonography and postoperative pathological results:among the 32 patients,6 showed positive cervical lymph node metastasis indicated by color doppler ultrasonography,while the other 26 showed negative cervical lymph node metastasis indicated by color doppler ultrasonography.All the 32 patients successfully completed radical resection of esophageal cancer and cervical lymph node dissection,without perioperative death.Postoperative pathology suggested cervical lymph node metastasis in 7 patients.Among the 6 patients with positive manifestations of lymph node metastasis indicated by ultrasound,83.3%were pathologically diagnosed as cervical lymph node metastasis after surgery,and 7.6%of the patients with negative manifestations of lymph node metastasis were pathologically diagnosed as cervical lymph node metastasis after surgery,with an accuracy rate of 93.7%.Conclusion:1.Color Doppler ultrasound has the advantages of high accuracy,high sensitivity,low misdiagnosis rate in the diagnosis of cervical lymph node metastasis of thoracic esophageal cancer.It has positive significance for the choice of cervical lymph node dissection,and can be used as the thoracic esophageal carcinoma.The preferred method of imaging examination in lymph node assessment.2.Three-field lymph node dissection was recommended for patients with thoracic esophageal cancer who had a positive cervical lymph node color before surgery,a tumor location in the upper thoracic region,and a preoperative T stage T3.
