Objective: To study the bureau within a small cell lung cancer(limited stage small cell lung cancer,LS-SCLC)1 year postoperatively in patients with distant metastasis of risk factors,mainly in clinical situations,smoking history,family history of tumor,tumor size,lymph node metastasis of biggest,clinical TNM staging,surgical approach,cycles of chemotherapy,preventive radiotherapy,radiation in preventive cerebral lymphatic drainage area,explore its correlation with distant metastasis after one year.Methods: The clinical and follow-up data of LS-SCLC.patients treated by surgery from May 2013 to May 2016 were retrospectively analyzed.According to whether distant metastasis occurred within one year after surgery,they were divided into distant metastasis group and non-metastasis group.The general clinical characteristics and related risk factors of the two groups were compared,and the risk of distant metastasis within 1 year was predicted by multiple Logistic regression analysis.Results: 272 patients were included in the whole group,with 42 patients with distant metastasis(15.4%)and 230 patients without distant metastasis(84.6%).The most common distant organ metastases were brain in 9 cases(21.4 %),bone in 6 cases(14.3percent)and liver in 5 cases(11.9 %).Distant lymph node metastasis was mainly in 5cases(11.9%)with supraclavicular lymph node metastasis.There was no statistically significant difference between the two groups in the maximum size of tumor(?5cm and >5cm),the proportion of lymph nodes not removed,the proportion of precautionary brain radiotherapy and the surgical methods.Distant metastasis group of mediastinal lymph node metastasis,and more clinical stage ? b or higher,pure chemotherapy and chemotherapy cycle < 4 group were higher than the transfer,while the former lymphaticdrainage area less than preventive radiotherapy of metastasis group(all P <0.05).Logistic analysis showed that mediastinum multistation lymph node metastasis(OR = 2.058,95% CI1.465-2.058)and clinical stage acuity ? b(OR = 4.247,95% CI2.134 7.482),the pure chemotherapy(OR = 3.372,95% CI2.258-3.372)and lymphatic drainage area preventive radiotherapy(OR = 1.658,95% CI 1.658 to 1.658)are independent risk factors for distant metastases occur within 1 year forecast.Conclusion: 1 age,smoking history,family tumor history and tumor maximum size are not important factors influencing distant metastasis of patients with small-cell lung tumor surgery after.Stand more than two of mediastinal lymph node metastasis,clinical stage acuity ? b preventive radiotherapy,chemotherapy alone and lymphatic drainage area are independent risk factors for distant metastases occur within 1 year forecast;Evaluation of regional lymph node metastasis,clinical staging and standard postoperative adjuvant chemoradiotherapy can reduce the risk of distant metastasis within 1 year after LS-SCLC surgery.
